Episode 1
A pair of new paramedics, Omar and Tamzin, start work alongside Dixie and Jeff. A pair of drug addicts hijack a car and create Casualty Car Crash 268 by turning it on its roof at the climax of a chase by cops. Both occupants sustain injuries. The passenger is pursuing girl cop Simone's kid brother Paul who dies in Emergency Department. Cop William Paynter gains entry to the home of drug dealer Colin Danvers and inflicts injuries on him and a punter, JC, at the property. Both gents are taken to Emergency Department. Nick Jordan has a nose bleed which makes him think his brain's gone wrong again so he goes for a scan. Dixie and Jeff are called out to the farmhouse of local drug lord Norman Danvers (father of Colin) who's had his leg mauled by a newly acquired German Shepherd dog. Jeff also takes a mauling from a second dog who's just eaten his sandwich. Cop William also comes to the farmhouse. Omar, Tamzin and Simone come to the rescue and manage to trap both dogs in an ambulance. Nick gets the all clear and becomes even more unbearable. Dylan teases Jeff about rabies.

Episode 2
Another male and female duo of new starters, Lloyd Asike and Scarlett Conway, are introduced to Emergency Department. Harry Timms starts work teaching at a school for special needs students where asthmatic teacher Emma Davies lashes out at student Lee Stark who goes berserk and crashes through a glass door sustaining serious injuries while smashing up a classroom. At Holby, Lloyd steps up to do chest compressions on a critical patient of Nick's - the patient lives. Stunning Scarlett Conway immediately receives relationship interest from gentlemen and compliments from a drunken female patient, Grace Fitch, who has various cuts. Scarlett deduces she has self harmed in order to gain the attention of her father, professor Fitch, in the hope he will stitch up her wounds in person. He declines. Ruth and Scarlett do the sutures. In Emergency Department, Dylan treats Lee and talks to Lloyd with the utmost, and most amusing, disrespect. Emma uses emotional blackmail to get Harry to say she didn't hit Lee. Harry tells Lee this and Lee goes berserk and smashes up hospital things. Lloyd counsels Lee - unfortunately he can only interpret the advice as to do more harm. Back he goes into the operating theatre when injured by the glass shard he hid in his pocket. Harry retracts the false statement and makes amends with Lee. Charlie tells Jay he is to mentor the newbies. Jay is frightened.

Episode 3
Several patients come in with the same symptoms and start dying. Dylan is scrambling to find a common factor before they all die.

Episode 4
A woman is convinced that someone is stalking her. Lloyd refuses to lie to a patient.

Episode 5
An elderly man loses control of his mobility scooter, goes down a hill, and crashes into a tree.

Episode 6
Annie and Chloe return to the ED as Annie has a sore leg, but Chloe thinks her friend is taking drugs again.
Episode 7
Jordan closes the drug program and Chloe's drug dealer beats her. She is in critical condition.

Episode 8
A girl passes out on a bridge and falls onto Charlie's car. He takes her to the ED and she claims that she keeps seeing angels.
Episode 9
Sam Nicholls, a former Army medic joins the ambulance staff.
Episode 10
Dylan is surprised to see Sam working as an ambulance attendant. He has a past with her.
Episode 11
Dixie gets a feeling that she is being used to chauffeur an elderly man around by frequently calling the ambulance.
Episode 12
Dylan discovers that a mother has taken and instant dislike to her son and he tries to find the cause of it.
Episode 13
Ruth is worried her medication is no longer working and discovers this is because she is pregnant by Jay. Patients include a social worker knocked down by a car while chasing a young mother and child and a very poorly man who wants to die but whose wife doesn't.
Episode 14
A tramp named Jock ends up in the ED for fighting with another "man of the road" over a woman.
Episode 15
An unpleasant face from the recent past returns to take revenge.
Episode 16
As the ED is consumed by fire, four staff and a young girl are trapped inside. Ruth and Jay say goodbye.

The crew start work at their new ED in Cardiff; a multi car pile up and a gas explosion which uncovers dangerous chemicals brings in many casualties.
Episode 18
Jordan is attracted to the new police chief Yvonne. Doctor Tom Kent joins the ED.
Episode 19
Linda's sister turns up at the ED with her two children, looking for sanctuary.
Episode 20
Linda's sister fails to return for her children, leaving Linda in a terrible predicament.
Episode 21
A woman faces a decision: continue to care for her mother, or put her in a care centre.

Episode 22
Dylan and Zoe are concerned that Sam is getting far too involved with a patient.
Episode 23
Dixie is overwhelmed by the zealousness of the new paramedic Rossy. He is too good to be true.

A formal complaint is made to the GMC about Sam.

A group of fools are admitted to ED with LSD overdoses; a wannabe gangster shoots a young boy.

Pleasure becomes business on Nick's first date with Superintendent Rippon and Linda and Zoe's night out. A man with learning difficulties attacks Lloyd.

Sam and Dylan go to the rescue of a couple who have fallen down a deep pothole; Linda's scallywag niece makes another appearance.
Episode 31
Teenager Alicia Dans returns to the ED after collapsing at school, but Linda suspects her problems are psychological and that she has developed an unhealthy fixation with Tom. Meanwhile, a woman takes extreme action against her nuisance neighbors, and Lloyd is upset by a racial taunt.

Episode 32
Two sisters are treated by Sam, but she can't focus because of the upcoming GMC hearing.
Episode 33
Sam finds out that an old army friend is going to testify against her in the GMC hearing.
Episode 34
A young boy in the care of the ED dies. Surprisingly, his parents are under investigation for it.
Episode 35
A man on medication for sciatica crashes his car en route to see his hooker; Dylan is being horrible to estranged wife Sam since the medical hearing and is further upset by not being invited to join the quiz team.
Episode 36
When a high school roof collapses, Tom is reunited with a troubled teenage girl whom he has previously recommended for counselling. But the girl's jealous boyfriend claims she doesn't want Tom treating her, so Jordan pulls him off the case for his own safety. Meanwhile, Linda thinks that she's making progress with Joe and Britney and they're starting to behave like a real family, until a furious Britney delivers a mighty blow.
Episode 37
An attractive but vicious lady loan shark is responsible for three casualties; Linda has to babysit niece Britney while working with help from Lenny, but she repeatedly gains access to the internet and meets an inappropriately older male.
Episode 38
On Lenny's last day, he and Linda go in search of Britney who has been taken by a paedophile.
Episode 39
New boy Fletch ruffles Lloyd's feathers; her transfer back to the army having not worked out, Sam returns much to the displeasure of Dylan.
Episode 40
The ED staff handle the fallout of a fatal breakdown of communication in the course of a police initiative on a crime-ridden housing estate.
Episode 41
Following the death of Wesley Royce while under arrest rioting and looting ensues.
Episode 42
Nick's cop girlfriend is seriously injured in the rioting. Scarlett says goodbye.
