With two engines on fire, the flight crew somehow manages to turn the crippled plane around to land in Honolulu. And though investigators immediately suspect an act of terrorism is to blame, the search for answers ultimately leads to a deadly short circuit.

Deadly Departure

A DC-8 with only three working engines is being ferried from Kansas City to a maintenance facility for repairs. The crew attempts a three-engine take-off, but the plane veers off the runway and crashes, killing all three pilots. Investigators dig into the pilots' training and discover that the simulator they trained on would have given them a fatal misunderstanding of the three-engine procedure.

And though they somehow manage to turn back towards the airport, the plane nosedives into the waters off Cape Cod-just minutes after takeoff. The wreckage leaves investigators with little to go on. But analysis of the flight data eventually turns the case on its head.
