After Cartman receives $2 from the tooth fairy for his lost tooth, the boys hatch a plan to get rich by stealing all the lost teeth they can find. Their plan goes awry, however, when it's revealed that there is already a tooth fairy mafia operating in the neighborhood, and the Godfather doesn't like these newcomers muscling in on his action. Meanwhile, Kyle begins to doubt the reality of existence and grows steadily more introspective until he finds himself becoming one with the universe.

During a playground argument, Cartman throws a rock at Token. Because Token is black, the incident is treated as a hate crime and Cartman is sentenced to juvenile detention hall where he must rely on his special talents for survival. Meanwhile, the other boys decide they have to get Cartman released when it becomes obvious that they need his bulk in order to win a sledding contest against a team of girls.

Although he is obviously intellectually disabled and is only capable of saying his name, none of the teachers recognize this fact and instead diagnose him as having ADD and give him Ritalin. As part of Timmy's treatment, he no longer has to do homework, so naturally all the other kids demand to be diagnosed with ADD and given Ritalin, too. Meanwhile, Timmy ends up creating a local controversy when he becomes front man for a garage rock band.

After seeing a troupe of Romanian quintuplet acrobats perform at Cirque de Cheville, the boys decide to form a circus troupe of their own. That night, however, the five girls and their grandmother sneak out of the circus and end up at Stan's house hoping to defect to the United States. After the girls' grandmother dies (following a night of heavy romance with Stan's grandfather), the boys convince them to stay in America while the Romanian government pressures the US to send them back. Meanwhile, Kenny tries to become a singer for the circus and ends up traveling to Romania as a musical sensation.

Tired of the abuse he suffers at the hands of his friends, Cartman decides he needs to hang out with more mature people, and goes on-line searching for men who like young boys. After his first two meetings end up being raided by the FBI, Cartman asks Dr. Mephisto for help. The doctor invites Cartman to join a group called NAMBLA, where he'd be more than welcome. Meanwhile, Kenny learns that his mother is about to have another baby, and sets out to kill his potential rival before he's born.

After Kyle gets sick, his mother hears about spiritual remedies from Stan's mother, and so begins taking Kyle for one quack remedy after another (from the new spiritual therapist in town, Miss Information) instead of getting him medical treatment. When she finally does take Kyle to a doctor, he tells them that Kyle's kidneys are failing and that he will need a transplant in order to survive. Unfortunately for Kyle, the only suitable donor is Cartman, who refuses to help him. Meanwhile, Mr. Garrison, having been fired for soliciting little Cartman for sex in the previous episode, decides to write a romance novel, "The Valley of the Penises."

Chef and Jimbo are debating whether the flag of South Park, which depicts four white figures lynching a black figure, should be kept. Jimbo wants to keep the flag out of tradition, while Chef feels it's a racist symbol. As the debate spreads through the town, the mayor backs out of making a judgment, saying the issue will be decided by a debate held by the schoolchildren. This pits Wendy and Cartman, who want it changed, against Stan, Kyle and Kenny, who don't see anything wrong with the flag as it is.

Cartman decides to realize his dream of getting rich by starting up a boy band called Fingerbang. The band, consisting of the four boys and Wendy (dressed as a boy), sets out to produce a video and get a performance gig at the local mall. Meanwhile, Stan's father has a fit when he hears about Stan joining a boy band, revealing a painful secret about his own past.

Father Maxi, the local Catholic priest, convinces the boys that anyone who doesn't confess their sins in church will be condemned to burn in hell. Concerned about Kyle and Timmy, (not to mention themselves) the boys become even more religious, especially after they see Kenny get hit by a bus. Their faith is shaken, however, when they see how Father Maxi behaves on his personal time, and the boys decide to start their own religion. Meanwhile, Satan is caught in a tangle between Chris, his new and more sensitive lover, and Saddam Hussein, who has returned after being killed in the South Park movie.

In the second half of this two-part episode, Chris and Saddam Hussein are violently battling each other for Satan's affections, while Satan becomes increasingly confused over which one is right for him. On Earth, Cartman's new church is receiving telephone calls from Kenny (who didn't die, but just ended up in Mexico), which Cartman uses to scare the kids with images of the afterlife.

The children begin their first day of fourth grade and are dismayed when their new teacher, Ms. Choksondik, is far stricter than Mr. Garrison was in the third grade. Hoping to return to those happier times, the kids enlist the help of some amateur scientists to construct a time machine that will send them back one year. Meanwhile, Ms. Choksondik, desperate to find a way to make the children listen to her, searches for Mr. Garrison, who is now living as a crazed hermit in the wilderness.

Mr. Garrison, having been re-hired as a kindergarten teacher, starts off the year by holding an election for class president. The two contenders are Ike (Kyle's little brother) and boy named Filmore. Meanwhile, a strange man shows up at school claiming he's a robot from the future sent to destroy Cartman's new Trapper Keeper.

Every year for Thanksgiving, the fourth grade puts on a production of The Miracle Worker. This year, however, they face stiff competition from the kindergarteners, who are putting on a highly professional show with pyrotechnics and musical numbers. Desperate not to be shown up, Cartman decides to turn the show into a massive musical production finishing with a stunt turkey leaping through a ring of fire. Unfortunately, Timmy (who is playing Helen Keller), has adopted a hopelessly untalented turkey as his pet and refuses to let it be replaced.

Presented by "A British Person", this episode looks at the background of the character Pip, in the form of a South Park version of "Great Expectations". Young Pip, having first been employed in the service of Miss Havisham, an insane heiress, learns that an unknown person has paid to have him sent to a fine boarding school so that he may be raised as a gentleman. After finishing his schooling he returns to visit Miss Havisham, whom he believes was his mysterious benefactor. Only now does he learn what the woman's true intentions were, at which point the story separates completely from the original Dickens novel.

His friends appreciate the change at first but soon find themselves wanting the old Cartman back and becoming suspicious that the new one is not what he appears. Meanwhile, Kenny becomes a minor shock-celebrity after agreeing to perform a series of increasingly disgusting acts for money.

The plan works perfectly and soon all the kids want their parents taken away as well. A few weeks later, a pair of lost travelers stop in South Park looking for repairs, only to find what the town has turned into now that the children are in charge.
