Two young girls are injected with the original Tomie's blood. As they grow up, they both become apparent copies of Tomie. After a chance encounter in childhood, the two girls become rivals. Several years later, a young man called Kazuki suffers from depression and PTSD after witnessing the brutal murder of his girlfriend Naoko and the mysterious disappearance of her dead body. His co-workers introduce him to one of the two Tomies, who seems to like him. She is a lookalike of the deceased Naoko, but Kazuki feels no attraction of her. The young woman thinks that he is playing hard to get, and she is soon obsessed with her would-be-lover. The rival Tomie seems to develop similar feelings fir Kazuki despite barely knowing him. He becomes the unwitting prize in a vicious struggle for dominance between the two rival Tomies.
