When Chicago Med's new emergency room opens, an L train derails and there are a lot of injuries that are brought to Chicago Med. And Connor Rhodes, a doctor who's on his way to Chicago Med is on the train. He goes in and takes charge which rubs some people the wrong way. And among the people brought in is a man who suffers severe head trauma. A girl who was with her father but the father's not at the hospital. And a woman who suffered head trauma is revealed to be pregnant. They learn she's the surrogate for a couple. The couple is worried about the baby. They think she's comatose but when she shows signs of being aware, they want to operate on her but the couple fearing for the baby refuses to let them. And a boy who has Cystic Fibrosis who's been to the hospital a few times comes again. And Charles who bonded with him stays with him.

A young girl who is bleeding is brought in. While examining her, they discover she just gave birth. They search for the baby and find her in an alley. Natalie feels sorry for her and tries to help her, while Brian thinks what she did is a crime. Reese admits a woman who acts strangely. She calls Charles who thinks she has dementia. When her husband shows up, she doesn't recognize him. Halstead examines a man who came complaining of chest pains and he has him tested and nothing is wrong and sends him home. But he later comes back suffering a severe attack.

But they later what really happened. And a woman who was trampled all over is now brain dead. Her husband consents to donate her organs and she's a match for the boy who started everything. Nat treats a girl who she thinks is anorexic so she consults with Charles who doesn't agree with her.

Firefighter Christopher Hermann collapses and is brought in. He's being treated by Rhodes who earlier treated him for an injury who told Herrmann he's OK. His fellow firefighters all wonder did Rhodes let Herrmann go too soon. A woman who tried to commit suicide is brought in and when Charles looks at her things he doesn't believe she tried to commit suicide. So he asks Halstead to ask his brother to look into it. And they find a suicide note claiming she has cancer. But the post mortem reveals that while she was receiving cancer treatments she had no cancer. Later a woman is brought in who says she has cancer but no sign of cancer. And another woman brought in is also being treated for cancer whom they learn doesn't have. So they come to the conclusion that there's a doctor who tells his patients that they have cancer and is treating them. April's brother, a medical student starts working there and Reese takes him around and she eventually discovers he's inept, a thing which drives a wedge between April and Severide.

Rhodes treats a couple of kids who stowed away on a plane to enter the country. When the Feds show up wanting to see them, Rhodes tries to keep them at bay because the girl told him what she was escaping from. And he also has to deal with his father making a large donation to the hospital. And at a party honoring him, he talks about his wife who committed suicide. Although Rhodes doesn't think his father was telling the actual circumstances of her death. Nat and Reese are treating a boy who was brought in by his parents. Nat suspects what's wrong with him and when she gets confirmation, before she could tell the parents she goes into labor so she tells Reese to tell the parents and she asks Charles to join her when she tells them. And Will goes to Nat to support her but her mother in law is concerned about his intentions.

Meanwhile, Sarah Reese (Rachel DiPillo) finally learns about where she will do her residency, but has last-minute second thoughts.
