Back after a torturous cancer treatment, administrator Michael Ragosa has enrolled in medical school again, insists on joining a night shift and throws his resignation into his boss's face, only to regret that fast. Army veteran Dr. Joey Chavez makes a cheerful entrance. Drew and on orderly mate's insistence at discretion costs them cruel pranks. An Asia couple is brought in, impaled together.

Back at the Ranch

Back in hospital following a brief suspension, TC must pronounce a woman who was accidentally shot by her rancher husband dead upon arrival. Drew is desperate to get the uncooperative Veterans Association bureaucratic doctor to sign for gay partner Rick to get a prosthetic, until he finds the right interlocutor. Ragosa starts his new job as a physician's assistant, countering hazing temptation with remarkable knowledgeablity, while Topher and Jordan ponder who should get Ragosa's old job. Kenny helps a star high school football player deal with the fact that his injuries will likely mean he can't play contact sports anymore, sharing is own experience which lead to his nursing career. Nusre Krista and cheeky veteran doc Joey Chavez seem to connect, to Kenny's annoyance.

An eloped couple of newly wed teens is badly wounded in a car crash, like the pregnant guilty party. Bride Taylor's father, war veteran sheriff Denny Sawyer, turns bitterly on less-injured 'spoiled' groom Ryan Harrison and his rich parents, but gets tragically out of control himself. Michael Ragosa is duly hazed as 'new' resident. Dr. Topher Zia dumps a malingering crone on the hospital board on resident Paul Cummings, whose gentle manner works wonders. Scott is coming to terms with Jordan having chosen unruly TC over him, yet suspected of prejudice.

Shock to the Heart

While TCs team operates on a skydiver who crash-landed on a bus, Topher alerts them that he has signed a DNR, which the girlfriend has no knowledge of and contests, indicating that the jump the skydiver made was intended suicidal. Student Kyle bring to ER a female college mate who claims she must be Ok but faints on the floor. Finding a tampon that the girl forgot was inside her, Paul suspects Toxic Shock Syndrome. Tests show semen on the tampon, leading the girl to believe she was drugged and raped at a recent frat party she attended. TC and Jordan tend to a Mexican mom and her children, whose husband/father came to the ER with a mysterious ailment. Soon, the mom and children also come down with illnesses that have different symptoms such as vomiting and collapsing on the floor. Acting as interpreter and host for the Spanish-speaking family, Ragosa later finds evidence in the father's truck that his family works on a farm that uses illegal pesticides. Drew's boyfriend Rick comes to the hospital to treat his ankle he sprained trying to change a light bulb and complains of pain in his prosthetic leg. Feeling depressed, he tells Drew he doesn't feel like the same person anymore, and wants to move out.

Ghosts

A costumed zombie run brought merriment and craziness, but also floods the hospital with patients. With T.C. leading a team in the field, Topher remains in charge and lets Paul handle a construction worker, only to discover last-minute his atypical wounds aren't accidental but radiation, requiring a quarantine lock-down. Drew, posted outside for triage, manhandles any apparent threat to him and the cop partner of a shot police hero. Chavez is delighted with a field assignment, actually with an overseas NGO and gets Jrista highly interested, on top of their hot affair. Scott rescues Jordan, who chose not to tell TC about her stalker patient.

Fog of War

T.C.'s daredevil driving on the way back from a medical event contributes to him, Jodan and Paul being stuck by car crash into a bolder on the road. They find a pre-crashed truck with wounded: illegal Mexican immigrants and the feared 'coyote' smuggler. They improvise and wit fro the fog-impaired medical helicopter, ultimately even argue whether TC should risk his own life by donating blood. In hospital, Drew is the victim of snapping by interim chief Topher, who suffers nightmares from his assistance to water-boarding in Afghanistan, while they must attend to a still-drowning knave who dived after his kid sister. Limber Joey Chavez demonstrates his yoga-skills and confides into Krista its tragic antecedents.

TC brings his sister in law to the hospital when she has an accident. Jordan learns that he's been hanging out with her and is not happy. She tries to tell him that she may have been taking something but he doesn't believe her. But Gwen tells him it's true. Drew is behaving erratically after his break up. Krista tries to tell Kenny about her and Joey but he tells her he knows and dumps her. And Gwen pursues him. Ragosa's ex-wife brings their daughter to be treated. His ex leaves their daughter with him and he tries to keep her from finding out what he now does.

The hospital is flooded with patients after a gas explosion set started major fires. Drew is charged with triage. TC's fat patient's concern for his badly burnt niece turns suicidal due to bad conscience. Scott stands up for Paul against his impossible to please father, but has to eat humble pie as a neurosurgeon his class is urgently needed, Paul resists paternal expectations to accept an alma mater residency.

A man who was stabbed is brought in. The police show up saying the man's daughter is the one who stabbed him. He doesn't want to press charges but the cop says it's not up to him. He says his daughter is mentally ill but the cop says even if she is she needs to be confined. And the father doesn't want to do that, So TC and Topher try to find out what's wrong with her. Paul gets a pent who is a boy who has respiratory problems which can be treated with meds but the mom can't afford them, so he tries to find a way to get them. Drew goes to a friend's promotion ceremony and Krista goes with him. during the party someone shoots a Colonel and a stray bullet hits Drew's friend's wife. So they are both brought to the hospital. The wife is exhibiting some strange symptoms which makes Drew think something else might be wrong with her so he runs some tests to find out. Ragosa is having trouble being able to pay for his daughter 15th birthday or quinceanera.

Aftermath

Drew feels press attention for his heroic rescue 'punishes his good dead' as he's now openly labeled gay doc, but gets a more pleasant surprise as his lover Rick Lincoln finally returns. Soldier and amateur Carpenter Brent Geisting is duly treated for a saw accident, but also diagnosed with a blood disease and an immunity-condition affecting his marriage. A corpulent woman's fat daughter's voracity is diagnosed with a surprising root cause.

TC goes to a rodeo where there was a structure collapse and he has a close call. He saves a man whom he brings to the hospital and Jordan treats him and discovers his behavior is strange. Paul and Krista are treating a girl and things get tense between because they both applied to be Chief Resident. Kenny goes on a ride along with Gwen, and they talk about each other but Gwen clams up when things get personal. They come across an accident were someone was seriously injured. The father of the one who was injured demands the one responsible be arrested and it's Scott. They bring him to the hospital and Scott wants to operate but the police show up demanding that Scott take a sobriety test. He has to wait for the results before he can operate but the patient doesn't have that kind of time.

Moving On

TC goes to a construction site where two men were injured. When he arrives one of them died and he treats the other who is frantic that his friend is dead. He later reveals that his friend was hurt because of a prank that went wrong. TC discovers the man has a condition which explains what went wrong with the prank and affect his life. Jordan treats a boy whose mother is trying to get her son released as soon as possible. When Gwen sees the boy's parents she suspects she knows what's going on with them. And how she reacts gets her in trouble. Ragosa, Drew and Krista take part in a wager with Kenny as the judge. A friend of Topher and TC's in the Middle East needs treatment which he's going to get but when Topher sees him online, he thinks he doesn't have time so he tries to go there but TC thinks he should be the one who goes but Topher won't let him because of Jordan.

Topher and TC arrive in Afghanistan to help Topher's friend but they learn he was not able to go to the base. So they go to where he is where there are a lot of insurgents. Back in the US, Ragosa is finally certified and gets his first case, a person who was in an accident who dies and he has to notify his wife. Paul because Krista said he doesn't go in the trenches like the others goes with Drew to the accident and when someone is trapped, Paul helps him but his hand gets burned which could affect his future as a surgeon. Scott is being sued by the family of the boy who was injured when he was driving. Jordan is treating a young woman who was injured in the accident and when Jordan thinks something is wrong with her, she refuses tests because she can't afford it.

Darkest Before Dawn

Drew is nearly shot by one of a pair of sniper terrorists in San Antonio, whom he brings in for treatment, yielding violent ingratitude, but one of his victims proves worse. TC and Topher bring in Afghan translator Ali for a cardiac transplant, but he'll lose the heart unless they can find and cure an unidentified infection. Scott, Chavez and Paul are back.
