San Antonio Memorial's administrator Michael Ragosa, whose ever-worsening eyesight wrecked his surgery studies, must run a rigid budget regime to avoid his money-draining ER being closed down by the board. Dr. Jordan Alexander gets a chance as new chief of a staff consisting mainly of war veterans, like her. Brilliant surgeon TC Callahan, who was fired from the Afghan war zone for insubordination and sleeps drunk in a friendly deputy's jail, seems most unlikely to bend for the pair of them. Ragosa, whom TC first decks for opposing excessive pro-bono treatments, wins TC's respect by instructing a novel skull surgery procedure for pre-teen Matthew, whose head was nearly severed in a crash while his devoted brother Tommy drove.

T.C. learns that a man he saved by giving him his dead brother's heart has been shot in the chest. He learns that unless he gets a new heart he will die. And when an addict Topher didn't want to treat suffers severe head trauma, T.C. wants to use his heart for his friend. But Jordan reminds him of protocol. T.C. goes to the addicts parents. Drew treats a boy who injured his arm while riding his bike. But he suspects something so he has the boy admitted so that Landry can evaluate him.

As Drew (Brendan Fehr) struggles to hide an injury, a series of accidents leaves TC (Eoin Christopher Macken) reattaching a hunter's severed hand and lands Jordan (Jill Flint) in the middle of a family feud that threatens to stop a lifesaving blood transfusion.

When participants of a reenactment of the Alamo are injured they go to the ER. And one of them shoots a man, who is brought in but has a bracelet that signifies he's part of a sect that refuses medical treatment. Jordan removes the bracelet and proceeds to treat the man. One of the paramedics who brought the man in informs Ragosa of his belief. That's when he removes Jordan from the case, and they try to have him declared mentally incompetent. So Landry tries to talk to him but he's unconscious. T.C. wants her to declare him mentally incompetent to help Jordan but she refuses until she has a chance to assess him. And it's while they are talking Jordan sees them and learns about them. Paul makes a mistake that nearly harms a patient. Drew is away for his Army reserve commitment and during an exercise there's an accident. One of them needs immediate attention. And their vehicle is damaged and they can't communicate so they have to carry him to where he can get help. And Ragosa has a headache and finds a bottle of what he thought was aspirin on Landry's desk and takes them but he later learns they're not aspirin because he starts acting weird.

A series of storms leaves Topher's (Ken Leung) pregnant wife and their daughter stranded on a highway just as she's going into labor. TC (Eoin Christopher Macken) squares off with Jordan's (Jill Flint) boyfriend (guest star Scott Wolf) as he's being courted to head up the hospital's trauma department.

New trauma chief Dr. Scott Clemmens planned to reorganize the ER, but all hands are needed after an army bus crash floods the hospital with injured soldiers from Drew's boyfriend Rick's Afghanistan unit. Among them are pleasantly-cheeky virgin private Wilson, who flirts with Krista, but whose condition worsens dramatically, and Drew's boyfriend, captain Rick Lincoln, who claims preferring death to the leg amputation that Clemmens insists on; Rick also isn't amused that Drew stubbornly refused to be outed by openly holding his hand. TC fights Scott for a Hail Mary alternative therapy, which fails in a similar fashion to Jordan's attempt at avoiding telling her lover the romantic truth about already mutually hostile TC.

A crashed drug-smuggle airplane arouses DEA attention while TC, who's haunted by his brother Thad Callahan's death while both served in Afghanistan, and Topher find one survivor. Agent Milo Osbourne stops at nothing to interrogate him about missing cargo. Worse, he's actually a fake cop and kidnaps the doctors to force them at gunpoint to make him talk instead of performing urgent surgery, making his point in a senselessly lethal way. A pole dancer who fell off is left in rookie Paul's charge while waiting for test results, but when he pays her for advice about girls she faints during his free lap-dance. This remains unnoticed long as everybody is flooded with victims from a major industrial accident.

With Topher's (Ken Leung) life hanging in the balance, TC (Eoin Christopher Mackin) is haunted by memories of his brother's death in Afghanistan. Jordan's (Jill Flint) treatment of Dwayne's killer puts her and the hospital on shaky legal ground.
