"The Big C" is retitled "The Big C: Hereafter" for its fourth and final season which finds Cathy (Laura Linney) winding down. Adam (Gabriel Basso) turns 16 which prompts Cathy to throw him a party. Andrea (Gabourey Sidibe) is mentored by guest-star Isaac Mizrahi at fashion school. Cathy checks in to hospice where she's visited by Dr. Sherman (Alan Alda). Paul (Oliver Platt) reunites Cathy and her estranged father. Sean (John Benjamin Hickey) decides to be charitable by donating a kidney. Adam fulfills one of Cathy's dying wishes.
