Carter treats an abandoned baby in Chen's care, which superstitious rumor ascribes healing powers to, and lets Abby rope him in sneak in and replace Luka's aquarium, which she accidentally smashed while clearing her stuff out of his place, but they are reported and arrested till Kovac refuses to file a complaint. Weaver fires Malucci after she finds him in a compromising position and he calls her out before leaving the ER in anger.

One patient refuses to undress for examination, another is beaten by a debt enforcer. A Latino's wife dies because they followed medicine instructions in English mistaken with the Spanish sense of 'once'. Corday lost a patient to an odd infection she may be spreading, so is suspended and subjected to a testing battery of tests. Abby bristles at a newly emerging relationship in the ER.

Green has to collect his daughter at school when she is suspended.

Luka tells Abby that he may marry Nicole, who has told him she is pregnant. Dr. Green lays down the law when the police bring in a high school student and he learns that his daughter is outside sitting in the police car.

With his grandmother still recovering, Carter is pleased that his father will be spending Christmas with them. He's not too pleased when he learns why his mother won't be joining them. Abby learns that Nicole may not have been lying to Luka after all. Green treats a young boy who was accidentally shot by his mother.

Abby also has to deal with a little boy who spent the night in the waiting room when his mother came into the ER for treatment. Her ex also lets her know that he is going to remarry. Carter's mother reappears but it's not a happy reunion. He also meets Jing-Mei who thinks she knows why Kerry Weaver didn't answer her page. Mark Green thinks his daughter may be smoking. Kerry Weaver and Sandy Lopez go out on a second date but argue when Kerry is uncomfortable at the prospect of being outed.

Carter plays intermediary between his divorcing parents but his father insists the marriage is over. He also treats a rising rapper who won't accept that he is HIV positive. Kerry Weaver and Sandy have an argument about Kerry's refusal to come out.

Mark faces the awful truth about his vixen daughter after Ella is rushed to County for what he fears may be a drug overdose, having swallowed a pill or two from Rachel's backpack, while Corday appreciates what Kerry does to help Ella but has no love for Dr. Babcock. Abby puts herself in the line of fire by taking in her battered neighbor. Weaver takes the low road to punish Lewis for accepting a promotion, as Romano decided to let her take over some of the paperwork. Chen decides it is time to lose the gloves in her fight against duplicitous Kerry, but Carter refuses to side against his boss. Student Michael Gallant keeps doing menial non-medical favors for patients, at his peril.

While trying to heal old wounds Carter's mother just reopens them and is forced to relive a tragic personal loss while tending to a young cancer patient but it also helps Carter and his mom understand each other better. Also a string of food poisonings cause Susan to flex her muscles as the newest member of management.

Dr Greene returns to New York to find out his brain tumor has returned, is inoperable and his life expectancy is 5 months. With this and Elizabeth moving out heavy on his mind, he turns his focus to work. Dr. Chen is takes a tumble when Frank and the newly returned Jerry have a fight. In order to keep his tumor at bay, Greene undergoes a procedure with a gamma knife, continuing to hide the return of his tumor from Elizabeth. Abby discovers the man who attacked her was released, partially due to Luca intervening. A little girl confesses to Weaver that she shot her dad because he wouldn't let her watch TV. Susan takes care of Mark in Elizabeth's absence as he recovers from the procedure.

Secrets and Lies

Chief Weaver can't handle staff looking through a dominatrix's bag and a dildo put in her own locker, so sends Carter, Luka, Lewis -who tries to take the blame- Gallant and Abby to a sexual harassment seminar. It's a snowy morning, so the therapist arrives very belated, leaving the 'trainees' bickering and exchanging memories and 'secrets', like when their cherries were picked, Luka even waited till his wedding night. Carter denies being silver spoon-spoiled, Gallant objects to disdain about his family being near-all in the military, Abby claims doctors are a haughty class but don't give patients proper personal care. Carter finds a fencing sword, Luka also had experience, so they 'duel', which turns nasty, apparently still over Abby, the major case of incomplete love affairs among staff.

Bygones

Romano forces just-returned Corday to work full ER shifts, where she nags Lewis, guessing she holds the key to estranged Mark's disturbing behavior. Lewis refuses to divulge Greene's secret, clueing Corday to figure out his tumor is back, but he tells her not to return pretending they still have a marriage. After a brutal case involving sisters who don't like each other, Corday gets some straight talk from Romano that sends her back to home with Mark. Greene reveals his condition to the hospital at large, Rachel figures it out herself. Weaver tries to keep it together after Lopez is reported lost in a school fire, but turns out to be alright, Kerrie visits her at a bar and they kiss. Two college girls are rushed into surgery, following a knife attack on campus that hits close to home for Weaver. After one of the victims whispers to her that she loved the other one, Weaver suspects a deeper relationship and that the surviving victim was actually the attacker. Abby moves in at Luka's while she searches for a new home.

Orion in the Sky

Training ignorant intern Gregory Pratt, who bumbles, even checking an unsettled ear pain patient's genitals in the wrong examination room, and back talks until he's told about Mark's tumor contributes Greene to a life-altering realization about the future of his own medical care and his ability to provide it to others: he refuses to sacrifice life quality to some prolongation, despite Corday's pleas. An obnoxious, liar junkie gets a lesson in life's hardships gone into premature labor, a museum worker displays signs of a legendary ailment usually attributed to the undead, and Corday must give in to a terminal patient's demands to die with dignity, enforced by er lawyer daughter after her shy son had her hospitalized. Mark Greene then refuses absurd treatment and finishes his last shift to spend his last days with his fractured family.

A frantic phone call sends Lewis racing to New York, where she enlists police officers Faith Yokas and Maurice Boscorelli to locate her missing sister and niece. Back at County, Carter confronts Abby about her drinking, but fails to sell her on a sober life. Corday gets bad news from Rachel concerning cancer-terminal Mark in Hawaii. Cocky Pratt annoys Chen and his showboating puts Gallant and a patient in peril, Carter must take control as head now Greene is gone.

Many struggle to deal with such loss, which affects patient care, as even Kerry can't handle clearing his locker for Pratt, while Carter remains remarkably composed, growing into his new role as post-Mark mentor of residents.

On last holiday in Hawaii, he takes his first and only parachute jump and takes Rachel on 'my history lesson' as USNavy base brat, then teaches her surfing and driving. He firmly resists Corday's insistence to get a check-up after a seizure, helps Rachel flirt with junior surf board shop clerk Kai and desperately tries to give her advice for life when her dad will be even more absent for ever. Many ER staff attends his funeral.
