Carter returns from his Atlanta rehab, eager to prove himself again, but not rostered again yet. Rowdy football player patients and mates resume rioting in the ER. Greene and Corday come back from their vacation only to find out they have a terrible rash. As her ex-husband did not pay her tuition, Abby cannot continue her ER clerkship. Chen is pregnant and about the point she can't hide it anymore.

Carter returns to County General and accepts Greene and Weaver's stringent terms for him to start half-time light duties under systematic supervision. In voluntary group, he already bumped into Abby, who he makes his 'temporary' AA sponsor; she clashes with most staff -but Mark supports her- while helping a couple whose too premature baby boy will die within hours, waiting for a priest. Greene braves Corday's domestic futilities wining a last time, then makes her year buying a bigger house and proposing. Luka can't abide by Kerry's legally obliged refusal to take a a lesbian partner's cue without formal authorization to make medical decisions on her behalf, so the estranged brother decides to intubate her as a plant. Benton can't get Romano to allow 28th surgery for a rude, lazy, therapy-neglectful patient whose insurance does not cover him, but instead of shipping him off as ordered 'anonymously' informs inspector Sandy Maggioso.

Dr. Benton has been written out of all the hospital's systems and is stunned by Dr. Romano. An unfortunate incident at a Science Fiction convention makes it look like scenes from the movie 'Mars Attacks'.

Kovac and Abby are attacked by a mugger with a steel pipe. Injured, Kovac protects Abby and smashes the robber's head on the ground repeatedly.

As the very pregnant Corday becomes increasingly agitated, the presence of her bickering divorced parents does little to improve her mood. The suspected shooter is his abusive father Derek Fossen, who strikes again before being brought in himself, and ends dying while Mark pretends administering vital electric-shocks. Carter is frustrated as his promotion application was simply waved and tells Abby to stop wining to him about 'too kind' Luka, who now entered her medical training application to avoid missing the deadline and offered to pay for it. Romano fires Legaspi for indecisiveness, Kerry threatens to resign with her and fight the 'lesbo-discrimination' all the way. Treating a HIV-positive victim, Cleo cuts herself and may be infected.
