Jerry also makes it, despite as selfish dragon mother complaining to everybody, mostly for her own 'trouble' like expenses. Steve's accomplice Rafe Hendricks prevents Sam escaping or leaving a message while buying sugar, speeding so as to get chased by a deputy, racing off till he topples fatally. After arguing how to proceed, Rafe and third accomplice Mary are fatally shot by Steve, who rapes Sam, falls asleep drunk and is shot by her.

Luke and Abby's baby Joe struggles in the NICU, Maggie visits and proves an usual help for Abby. Sam's former employer, Elliott, helps her forestall any police investigation of Steve's death. Pratt's joy to be promoted attending is cut short, having to teach the new interns and supervise the residents. Gates and Pratt's fight over Gates' failure to keep his charts current and the treatment of a gay heart patient results in his partner fighting his family over keeping him alive. Against Pratt's orders, Gates saves the patient's life, but he is brain dead, which ultimately is worse for all, especially Pratt. Gates has sex with what appears to be his roommate. After visiting Abby and baby Joe, Neela meets the new chief resident of surgery, Dr. Dustin Crenshaw, whose personality and scarce hair recall the hated late Dr. Romano.

Neela mentors contrary med student Jane Figler, assigned to surgery, and helps Pratt after a serious error, to the scorn of the chief surgical resident. Sam discovers Alex's latest extracurricular activity. Gates turns out to be living with Meg Riley and her daughter Sarah. Abby enrolls in a "Mommy and Me" class and hides being a doctor when she meets a group of selfish nannies, which makes her prefer minding he baby herself.

Luka is sued for 'malpractice' by carpenter Curtis Ames, who had a stroke -wrecking his health and family- while awaiting care for pneumonia, due to County's permanent shortages of staff, room and budget. During the trial, each man's recollections of Ames' treatment are recounted via testimony and flashbacks, Luka worries about the implications of the suit and the possible outcomes, yet insists Abby returns to work to adjust as a young mother. An equipment shortage culminating in a burnout sabotaging Pratt, Abby and Neela's treatment of a heart patient leads to attempting a risky procedure. Pratt's early morning trip to the barbershop results in a new awareness of community need while half-brother Chaz asks to move in as he can't study at home or find a decent room.

Luka must decide whether to allow a cancer terminal patient's wife to keep him ignorant to enjoy their last months abroad. Dubenko warns Neela after covering once she must submit to whatever chief surgeon Crenshaw commands. Pratt's romantic night is spoiled as Chaz moves in a day earlier then he expected.

Rude, aggressive combative patient Willis Peyton, who stormed out refusing vital treatment, returns later under a different name, a completely different person without memory of his earlier visit, Morris has a good idea of what's going on but can't get anyone to believe him until Sam steps in. Behind Weaver's back, Hope gives a destitute father free sample medication for a boy patient. Kerry refuses to become a regular TV medical reporter. Abby and Luka reconsider hiring a nanny.

Abby's friend from Ike's returns as destitute patient. Pratt celebrates when his moved-in brother brought a girl, but Caz fully comes out to have as 'pure gay'. Dixie new nurse Ben Parker proves as foxy as handy with trouble patients in the ER. Gates pours on the charm, Neela is not impressed as meg clearly still digs him, but Ray warns her she can't have it both ways.

Tony and Neela respond to Sarah's call for help and find Meg unconscious with an unknown overdose, terminal, and she tells Tony that he's Sarah's father.
