The ER prepares to take in victims of a plane crash into an apartment building, while Neela (still doing her mandatory paramedic stint) and Pratt, who just convinced grumpy old Martin Trudeau to resume taking his medicine and arranged a medicopter for a heart transplant, head out to the crash site because Pratt doesn't trust Ray -who is knocked down by an imaginary giant samurai patient- in the field. Neela distinguishes herself with a field surgery and a rescue, impressing Dubenko. Sam re-hires Haleh when the ER runs short on nurses due to a training session. Dr. Victor Clemente feels Luka doesn't respect his authority. Gallant's return surprises Neela.

One charachter has her last show but her leaving is well deserved. And Abby gives Kovatch not one but two presents. We have to wait and see the outcomes of some characters lives. A well thought out Christmas show and a special thank you at the end for someone that unfortunately passed away.

Clemente's past is starting to catch up with him as he grudgingly covers for lover Jodie Kenyon, who left her cop husband, who sent colleagues to investigate a missing person report on Jodie. Weaver fell on the ice and Neela must make her admits she needs surgery, so is grumpy when Ray asks for a month off as his band can get a Californian producer to make a record demo there, so calls his bluff when he says yo consider resigning for a shot as rock star. A teenage boy who left his 'meddling' parents without warning to be his own man needs surgery, having humped a tram for lack of cash to buy a ticket, refusing to return home.

Luka has due doubts when pregnant Abby drags him along for excessive baby shopping. Ray helps Zoe escape from her abusive father, way beyond medical care. Dubenko volunteers to take on the ER day shift but is overwhelmed and unable to say no, especially to an old man who will go to any length to prolong his very terminal wife's life a few days. Pratt falsifies a blood test, using his own, to help drunk friend Darnell Thibeaux avoid a drunk-driving arrest, but his son T.J. figures it out anyway. Neela insists to accompany Gallant visiting veteran friends, but is shocked to hear he decided to return to Iraq, feeling morally obliged, she feels their marriage vows should outweigh that.

Neela pulls an all-nighter in preparation for a speech at a surgeon's convention but is disappointed when her speech goes awry, fainting when falsely accused by activists, consoled by mentor Dubenko, and tells Ray she must move out. Morris' past catches up with him in the red-haired shape of his donor sperm kids. Sam accepts a part-time job as Richard Elliott's in-living nurse.

Luka tells Abby that Carter has asked him to join him in Darfur, but when he realizes Abby is worried, he tells Pratt to go and responds to Pratt's protests it's not a request but an 'or-you're fired' order. Sam enjoys the perks of working for Elliott. Ray struggles with some unresolved feelings for Neela who decides to move back to Abby's apartment.

Clemente wants to release Sam's ex-boyfriend Steve, who arrives at the ER complaining of a stomach pain, but Neela's pleas to keep him in the ER is proven correct: he has appendicitis. Morris wins an argument with surgery bitch Dr. Albright about a patient, and afterward they make out. After hip replacement surgery, Weaver no longer needs a cane. Pratt arrives in Darfur, greeted by Carter, quickly noticing the scope of the horror there.

Pratt and Dakarai are forced to make a journey to a hospital at the risk of getting stopped by the Janjaweed. Meanwhile, Debbie tries to convince Dr. Dakarai to get treatment for his medical condition.

Among patients brought in are Steve and another inmate who had a fight. An EMT shadowing Sam for the day turns out to be the other inmate's girlfriend and overpowers a police officer to free them, badly wounds Luka and takes hostage Sam and Alex, with a shootout when security gets suspicious cued by Abby, wounding a cop and Jerry, who protects the kid brought in with breathing trouble wearing a strawberry suit.
