Neela gets a surprise visit but little support from her domineering, old-school parents and Can't even get a menial job at County, so acts as immature Tesoro's travel companion.

Alex tells Luka he already has a father and Abby thinks new intern Howard may be obsessive compulsive.

Lewis takes the vacant ER chief position and offers Neela, whose sales job is humiliating misery, an ER internship at County.

Keith Trager insists to transfer his teen daughter, whose appendicitis diagnosis is stumbled upon to, out of the hospital which 'killed' his father.

On Christmas Eve the hospital is overrun by homeless people trying to stay warm, so Ray lures the 'profiteers' to the Holiday Inn, one fake patient robs the pharmacy. Pedantic Dubenko's secret Santa gift is too absurd to pretend accepting, Carter witnesses Kerry repackaging an unwanted gift for himself as she clocks out. When Chen can't find someone to cover her shift, as even Luka insists to spend his first Christmas with Alex and Sam at home, she quits, leaving Carter as the only ER attending. Carter and an off-duty Ray save a critically injured boy, and Ray learns the real story of his injury from his little sister. Abby and volunteering new medical student Jake Scanlon treat a dialysis patient who is in critical condition, but when she can't reach his attending, she breaks the rules to save the patient's life. Pratt heads to Chen's house and finds she is about to euthanize her father, a doctor, who has begged her in his lucid moments to do so and return his corpse to China.

Lawyer Herb Spivak shows up at the ER and tries to generate lawsuits against County, bribing staff with gifts, but ends up a patient himself. Pratt dazzles interns with speed and medical skills but struggles with his teaching role, as he rarely trusts any to try in order to learn. Abby returns to work while still recovering from her post-abduction trauma too soon and must walk off again, but med student Jake helps her deal with it.

Spending time together, Weaver learns the story of her birth, when Helen was 15, and unaware of her hip birth defect, which is why she needs a crutch. Abby and Jake adjust to their changed relationship. When Weaver reveals to her mother - an evangelical Christian - to be is gay, their meeting turns rocky. Jane impresses Pratt by handling an agitated patient and is the only one cooperating when Lewis seeks to collect data for a scientific publication.

Abby fails to prove that an injured, penniless 72-year-old woman is the survivor of elder abuse, so she and her sister are released to resume panhandling. Ray and Pratt seek a way to convince a penniless woman with breast cancer to seek treatment to avoid certain death.

Ray angers Carter by ignoring procedures when the elderly panhandlers show up in the ER again Morris may end up with the chief resident job despite a disastrous presentation, Lewis does her presentation for tenure. Sam takes a home pregnancy test.
