Ward 17 is business as usual as Bron heads off to help Lyle through the "opening night" nerves his play premiere has given rise to. But the best laid plans go horribly wrong when an earth tremor buckles train tracks and the train they are travelling on with 300 passengers on board jack-knifes hitting a tunnel pylon at high speed. Trapped in the damaged train carriage, Bron sustains a head injury but still tries to treat the wounded around her while attempts are made to free them from the wreckage. When the carriage starts to shift, the passengers are promptly buried under the carriage's collapsed roof. The tremor also hits Ward 17 and Von injures her wrist in a fall. With Von's arm in plaster, Ward 17 is understaffed and already at full capacity with patients but still everyone gears up for the influx of additional casualties from the train disaster. Dr Luke Forlano and the paramedics Ben, Scott and Rebecca attend the accident scene and all of them are shocked by the extent of the carnage. Things are made worse when they discover that Bron and Lyle are somewhere in the train. A gas leak triggers an explosion and not everyone gets out in time.

Terri and Mitch continue to dance around each other as the workload builds and more and more patients land on their doorstep with no more staff allocated to nurse them. And the tension between them is also rising. Luke and the trauma team set up operation at the accident site. Dom arrives, knowing some of his boys and one of the St. Anthony's teachers are on the train, and the lie that he is a priest comes a little too easily to his lips. It's his ticket into being granted special access to the accident site. Bron and Lyle are removed and sent to Ward 17 immediately. The boy trapped in the carriage doesn't fare so well and Dom struggles with his conscience on who has the right to immediate treatment - the boy or the elderly teacher. Luke amputates a man's leg in a train carriage, with Ben watching him in a new light. Ben witnessed Luke performing the same brain surgery earlier that he performed on him some months ago. With no operating suites available, Mitch is forced into performing life saving surgery in the ward. Terri becomes his support and the unspoken bond between them strengthens once more. On the train station, tempers flare between Rebecca and Scott, the latter still stinging from losing his patient earlier. Dom arrives at Ward 17 with the young boy and Paula realises he is the son of the man she's been dealing with. Father and unconscious son reunite but Terri is acutely aware of the fact that Dom is not celebrating. Her efforts to find out why meet with a wall and she realises things with him are nowhere near ideal.

It's Von's worst nightmare to see her patient Claire dying from the same disease, Lupus, that she has - she can't hide from the truth any longer.

Terri parallels her relationship with Mitch with this trouble on her ward which frustrates Mitch to a point where he takes matters into his own hands. Von and Nelson conflict over an elderly patient who is grieving for her recently deceased mother.

Driven by her fears for Mitch, Terri becomes irrationally determined to admit an elderly man so he can comfort his sick wife.

Bron finds he's hiding something. A wily outback type presents with a nail stuck right through his hand. His determination to fix himself leads to greater and greater complications. A young girl arrives in emergency suffering from abdominal pains and is surprised to be treated seriously as her mother almost never listens to her.

After making sure he gets his vaccinations, Von cleans up the house. Adam is still unsure of Von's intentions, but it seems a breakthrough of sorts has been made. Bron is gearing up for the wedding and all the while the secret of her overseas fling with a guy called Andy to keep from Ben. Jared asks Mitch for a job working in the new detox clinic but his recent personal drug problem undermines his chances. Mitch finds it impossible to cool things off with Terri.

The visit gives Terri a new insight into the childhood episode that shaped Mitch's life - the death of his sister Lucy. Mitch has to deal with a very determined Rebecca who is desperate to join the detox clinic's staff. Bron nurses a teenage boy recovering from a drinking binge who can't explain how his 15-year-old sister ended up unconscious, gravely ill, and sexually assaulted. Von attempts to persuade Adam's mum, Gina, to seek treatment for her heroin addiction. Dr Charlotte Beaumont replaces Mitch on Ward 17.

Having fixed a patient's surgical problems, Luke's prowess as a physician is put to the test when he has to find a medical solution for a Tourette's sufferer. Von is being sucked into the quagmire of a heroine addict's dysfunctional life.

Jared's personal crusade to save heroin addicts living in the underground takes a tragic turn when he loses one to an overdose. Kylie realises how empty her life outside work is when she treats a man completely at peace with the fact that he is dying of cancer. Ben makes a promise to Max and Paula spends the day tortured by her belief that Ben will disappoint him. Ben becomes reacquainted with widower Harvey Swaine. Charlotte faces Warwick Ellis' prejudice when she steps in to treat Harvey contrary to hospital insurance policy. When Luke operates on some of Bernie Farelly's fingers after an accident in his butcher's shop he is forced to deal with the five stages of Bernie's grief.

The future of Mitch's drug clinic is in jeopardy with one of the staff admitting to hiding their past addiction. Nelson asks for Teri's support while he is in charge of the shift.

When one of Mitch's clinic patients falls into a coma, he fears it may be another nail in the coffin of the clinic. Ben helps a pregnant woman to hospital who has conceived her baby from donor sperm donated by an ambulance officer. Ben is left wondering - could it be his? Jared is in denial about his feelings in the aftermath of his rape. In a jealous rage, Bron decides to do something that will shock Ben and everyone else in Ward 17.

Charlotte's broken heart is revealed when another doctor's husband is admitted to the ward. Rebecca feels that her relationship with Scott is going nowhere - until he says 'I love you'. Paula accuses Bron of being unprofessional, and stopping her from going away with Ben - but Paula learns that she is the one behaving like a child.

Terri returns home to find Mitch in a state of disrepair and she sets about getting him back on the rails. Charlotte and Jared are amazed when a 'professional' patient's chronic nausea seems to improve after being put on a drug trial. Nelson is appalled when he discovers a terminal cancer patient hasn't told her husband of her dire prognosis but he is forced to reassess his opinion when he learns her reason. Scott has told Rebecca that he loves her - why hasn't she said it back? Kylie learns she has been accepted for a new job in Paris.

Rebecca and Scott get caught in the crossfire of a bikie war. When a gunman appears on the ward, Mitch charges to Terri's rescue, and is almost killed in the process. A woman with a long list of unrelated symptoms is ear bashing everybody. Von finds the reason her young patient is withdrawn and self-mutilating is because he has a hearing problem.

Determined to give the pair a ritual purification, he holds Bron's head underwater while her former lover is forced to watch helplessly. When a sixty-five year old man has a massive stroke Luke sees an opportunity for a young woman to get the lung transplant she desperately needs. But the man's wife resists turning off his life support. What is the hold her husband has on her and why does she insist on holding his body hostage? The new ward clerk has Jared's full attention but Nelson sees through her act. Mitch discovers that Rose is taking him for everything in the divorce settlement.

Jared loses his patience while nursing a developmentally delayed patient. When Ian Neal escapes custody it has tragic repercussions for Ben.

Jared and Kylie work together on her last day to solve a patient's kidney problems, and end up solving Jared's problem too. Scott's suspicions are confirmed about his new temporary partner.

Nelson is offered a promotion as the new NUM in the Emergency Department but the day doesn't turn out the way he'd hoped when a highly charged ethical dilemma presents itself after a critically injured truck driver's two families arrive in the ED. His day is further complicated by the arrival of a mystery woman. Mitch is rattled when Rose makes a small but unexpected appearance, and is then confused by a puzzling case that Charlotte may hold the key to. Mitch becomes worried that he is no longer performing up to standard. Bron, who is replaces Nelson as 2IC, is trying to smooth the waters with Paula but only succeeds in making things worse. Ben's slow recovery in ICU is frustrating Bron, who wants to work through their problems.
