Grace treats a young female patient in desperate need of a heart transplant and, after one donor falls through, Grace learns that the girl's only hope is her estranged father. The only problem? The father, who Grace discovers is a former associate of mob boss Constantine Alexander (William Forsythe), is very much alive — and wants Grace to kill him so that he can donate his heart to save his daughter.
