Wedding bells may be ringing during the Grey's Anatomymidseason finale on Thursday (9/8c, ABC). But there are a few hurdles for April (Sarah Drew) to get over before she says "I do" to Matthew (Justin Bruening). First on April's list will be getting her best friend and former lover Jackson (Jesse Williams) to show up. But that does not mean the quirky doc is having second thoughts about walking down the aisle.

"She is 100 percent gung-ho and excited to marry this guy," Drew tells TVGuide.com. "The only thing that creeps into her mind is that, like she said in the re-proposal episode with Matthew, she'll always have feelings for Jackson. He'll always be a part of her life and he'll always hold a really important place in her heart. So, for this massive day, she wants her very important friend to be there for her, but he has not decided whether or not he's going to come or not, so that's the only thing that's messing with her emotionally about the wedding going off without a hitch."

April will also face challenges stemming from her decision to enlist Cristina (Sandra Oh), Meredith (Ellen Pompeo) and Arizona (Jessica Capshaw) as her new bridesmaids. "April is a little bit Bridezilla-y in that she's fired her sisters as bridesmaids and hired three people who are in the middle of some serious emotional trauma and also fighting with each other, so she does not have the most supportive bridesmaids ever," Drew says.

Fortunately, one thing that will turn out perfect for April is the actual wedding venue. "When we walked into the location for it, we were all blown away by the beauty of it and also by how much it screamed April," Drew says. "The setting of it is so right for her given that she grew up in a small town on a farm. There's a lot of that feel to it. There are little details that you'll remember from when she was dreaming about her wedding back last season when she had the pregnancy scare that show up in our actual wedding setting. It feels right for her."

Still, Drew is shy to say whether April and Matthew will actually tie the knot. "Nothing ever goes off without a hitch on Grey's Anatomy," she says. "You have to watch all the way to the end of the episode. It's full of surprises, especially the last five minutes of the episode are just five minutes you don't want to miss. There are lots of shocking things happening all over the place."

Part of the shocking final five minutes actually takes place back at the hospital, with Shane's (Gaius Charles) recent cockiness finally coming to a head. "The stuff with Shane is pretty crazy and is going to affect relationships once everybody is back at the hospital," Drew teases. "There are multiple pieces to the cliffhanger. We're hopping around during the last minute all over the place to the point that when we were reading it during the table read, Ellen Pompeo stopped the table read and was like, 'What?! What is happening? Are you kidding me?' It's not just April's wedding that is wrapped up into that last five minutes. We come back from the break with a hell of a lot of tension."
