Catherine Zeta-Jones has checked into a treatment center to seek help with her Bipolar disorder, TVGuide.com has confirmed.

"Catherine has proactively checked into a health care facility," a rep for the actress says. "Previously Catherine has said that she is committed to periodic care in order to manage her health in an optimum manner."

As first reported by TMZ.com, the 43-year-old Oscar winner checked into the center Monday and is expected to complete a 30-day program.

Zeta-Jones sought treatment for Bipolar II disorder in April 2011. She appeared on the cover of People days later to talk about her diagnosis. "This is a disorder that affects millions of people and I am one of them," she said. "If my revelation of having Bipolar II has encouraged one person to seek help, then it is worth it. There is no need to suffer silently and there is no shame in seeking help."
