The new chief of the ED Neeta Devi is on scene and gets mildly injured when a worker falls while working on wires, also causing an electrocution and car crash. Bash disagrees with Devi over a patient with DID, as she keeps putting rules over his pragmatic defense of patient interest, and is told unlike Bishop, she won't support him being instated as regular surgeon. Theo returns to the ED after the helicopter crash he was involved in at the end of season 2, but finds it hard to readjust and deal with 'escape luck guilt'. Mags must still adjust to her more senior position in cardiology.

When Bash finally meets Mags' francophone parents for the first time over dinner, Mags' advertising-employed pa Sylvain Leblanc chokes, only for Bash to save him using the Heimlich Manoeuver. After they had sex at her place, Bash still won't tell Amira, while Magalie and her mother worry if he is pulling back. Theo takes a reassuring blood test and helps Lola, who is afraid of needles by life-saving injection, but she later refuses him treating her further, implying 'assault', and the apprentice nurse sides with her, mentor Claire hopes she'll learn to see medical reality. Bash helps androgynous Otto, who is stunned to hear he has ovarian cysts and has to deal with his mouthy, meddling daughter, whom the new medical director insists to be involved, vindictively wining about her own ovarian trauma.

Bash struggles with Canadian sanctions against sending money to his friend Saleh in Syria, reminiscing a youthful trip there. Bash must work with a patient who took a 'controlled' dose of ketamine, undressed publicly and drives her partner to despair on the anniversary of their teen kid's fatal accident. June works with Dr. Singh in the Trauma OR while Dr. Novak is overseas for charity and offers to send the money to Saleh for Bash, while her half-sister demands urgent banking help. A patient of Theo's returns and is denied a kidney transplant, his attempt to get a critical patient's family to consent donating his if he dies is reviled. Singh invites 'apprentice' Bash to ignore Devi's stand-off instructions by assisting in complex surgery.

Bash's patient, veteran drilling instructor Gurjeet Sran, comes for a head wound, but has a deeper psychological trauma, which his therapist can't get him to tackle, so it's crucial that Bash react well when Gurjeet 'sees' in him comrade Nadir, who died in his wartime drama. Despite restrictions on his own actions, Theo helps patient Avrum's father Caleb deal with his son's condition. Mags recovers from her arrhythmia and requests Dr. Davi let her return to work as an ER doctor. June gets answers about Shay's whereabouts. Bash asks for help.

A 'needy' diabetic patient's teen son Griff has enough of her messing up his youth, but gets crushed between two vehicles after storming out of the hospital: it's up to Bash, Novak, Singh and Devi to save him inventively by working out a tight schedule. Theo is unwilling to avoid a lawsuit by apologizing for his alleged abuse. Bashir ponders how to punish Amira, who forged his signature to audition for a school. Psychiatrist Karim Esfahani helps him link his troubles to his Syrian youth. Head nurse Claire must tell Devi it's not possible to keep covering for all the consequences of under-staffing.

Bashir is worried-sick about runaway Amira and how to discipline the brat, so when hosting a dinner party for his doc colleagues looses his patience with ever-meddling lover Mags's unwanted suggestions. The chief Neil Olsen rebukes Devi for failing to put budget needs first by sympathizing passively with the nurses' by the book-action, while all doctors and their own chief struggle how to combine the legitimate fight for needed resources with patients' urgent needs, while taking over some menial chores themselves. An oversensitive adult patient reluctantly confides this conditions into Mags and lets him put through ample tests, but decides against invasive therapy.

Bashir has his last, decisive interview with immigration official Chandler, who clearly admires him but keeps asking potentially problematic questions about his Syrian horror past, when he risked his life smuggling a bone saw over the border, while probing about Bash's present hospital life. In hospital, conflicting expectations and suspicions from Novak, Devi, Mags and June keep challenging Bash. Theo asks him to translate from Arabic for scared teen Farid, who risks deportation if social services learn his family is undocumented. Bitter young patient Ezra needs an urgent lung transplant, but lashes out at his recently discovered twin brother Liam, whom he envies his good family. A recovered opiate addict grimly mistrusts painkillers and rages after receiving some in exploratory surgery.

Bash is finally allowed to take the citizenship oath, with Amira, who eagerly invites Mags to come along to New York when they get passports, but the relationships get strained when Mags reproaches his sabotaging insecurity as he still sees 'if', not when. Theo keeps clashing with Claire over how to apply legal protocols with a patient who tattooed 'do not resuscitate' on his chest. He wants to remain in control in order to practice minor climate terrorism, but his father has power of attorney to prevent him taking his own life, resulting in a court session by video. A couple was shot on their wedding day, the wife's condition requires urgent surgery under doubtful conditions, which Novak performs against Bash's advice.

Bashir has volunteered to help out as doctor in a predominantly Syrian refugee camp in Lebanon, where the cynical veteran in charge, Denise Garneau, explains the rudimentary means don't allow proper care: it's amputate a limb he could save in Canada or the patient dies. Loyalty issues prove even worse than in York, with youth buddy Saleh, who questions his motives and preparedness to risk a trip to Syria, as well as patients Bayan and Talia, who lie to him and each-other, trying to save the other's future at their own expense. Back home, Theo wants to go steady with Elizabeth Bergeron, but the adulteress can't decide whether to give her marriage with kids another chance. June is dismissive of a promotion as Novak still overrules her and she feels Olsen's abuse complaint handling by the book is a joke. The queues and waiting lists get so unmanageable when ransom hackers sabotage the ICT, so Devi closes ER to catch up.

In the Lebanese camp, Bashir not only fights for individual patients as always, but more assertively steps up when a cholera outbreak endangers many Syrian refugees, yet still finds time to make up with the youth buddy who had to betray him to save his own skin. In York, the hospital board turns on Devi for making medical choices beyond budget margins. A retired star doctor is treated for a complication but decides his terminal condition and loneliness justify euthanasia, which cold June seems perfect for. While Theo deals with being passed over for promotion by his adulterous ex, Mags finds her new choices as problematic as the old ones, but an innovative cardiologist seems right for her holistic treatment.

Theo is horrified to witness his climate-obsessed patient Campbell execute his suicide-threat by burning on the steps of parliament house, even worse o is surviving brother-guardian. Devi stand by Mags' attempts to control over hyperactive attitude to prevent a fatal cardiac crisis. June must perform doctor William Maxwell's euthanasia wish, both all alone.
