While working at an LGBT suicide hotline, Maura receives a distressing call from a young trans woman, Elizah. Afraid that Elizah will hurt herself, Maura goes on an odyssey through South Central LA to find the troubled teen and lands herself in the hospital.

Despite her collapse being a seemingly minor issue and her resulting hospital stay hours instead of days or weeks, Maura is concerned, not so much for what caused the collapse, but that if something serious had truly happened, she would have died a sad woman who did not fulfill what she wanted in life. As a side issue, she has no idea what happened to Elizah who could very well still be at risk. Sarah, who has moved back in with Len while he still maintains a sexual relationship with his millennial girlfriend, takes the first step to volunteer for the Board at the synagogue, Rachel who states that the meeting with the other members is only a formality. It may be a formality except in the case of Sarah, who is open to an extreme about her life. Josh immerses himself back at work with the new company, with one of his underlings, Seth, a seeming younger version of himself, sort of. Ali begins her teaching assistant job under Leslie. While the professional and personal balance between herself and Leslie may be difficult to negotiate in a practical sense, what Ali learns from and her dealings with the other TAs may make her life even more complicated. And Shelly gives a talk at the synagogue about how she has emerged from the depths of hell to a better place. After a talk with Buzz about it and receiving generally positive responses from those in the audience, Shelly has a notion of how she can parlay that talk into something bigger.

Relationships are tested as the Pfefferman family gather for Maura's birthday. Sarah confronts Buzzy on temple board politics and Maura makes a big announcement, experiencing pushback from both Vicki and Shelly. The night culminates in a game of Sardines, forcing tense situations into tight spaces.

The journey ends up being one of joint discovery for the two, the journey which hits some unforeseen bumpy patches along the way. With Leslie still at the house recuperating from her fall, Ali needs some space away from her, and ends up returning to a previous comfort. That return ends up being somewhat different, but revelatory nonetheless for Ali. And Buzzy is helping Shelly with her one woman show, "To Shell and Back". What Buzzy does, which has some ulterior motives, has some financial implications for Shelly.

Maura and Vicki go to visit Maura's mother in the hospital, Rose, in a coma, who probably will imminently pass. Instead of going to a restaurant to eat, Maura and Vicki take Bryna up on an offer to go to her place to eat, the acceptance of the offer based largely on Vicki's want. Vicki ends up being caught in the middle of the siblings, with Maura perhaps feeling like Vicki is siding with Bryna as an offshoot of their larger issues concerning Maura's want to have reassignment surgery. Josh reaches Colton's house. Josh has to decide how much about Rita he will divulge in light of Colton's religious beliefs which seem to be strengthening. In turn, Josh may be looking for some answers for his life, in light of his relationship with Raquel and what happened between him and Shea. And Sarah, who can see something between the two, urges Raquel to take a chance with Duvid. What happens demonstrates just how fragile Raquel is with her life.
