Padma put off an ultrasound, now finds out dangerously late her twins suffer a rare condition of misrouted bloodstream, causing one to get too much, the other too little, expected to kill both. Specialist Ian Sullivan recommends his signature surgery, likely to save at least one, only it requires rare equipment, so he refers to a Florida colleague, but heli-prohibitive weather and time pressure cause him to improvise. Dr. Sutton operates on the patient thinking she has a tumor, but it proves just a parasite. Conrad finally invites Cade over after Gigi's bedtime now she has recurrent nightmares about a strange man in her bedroom..

Devon has ethical doubts about aggressively treating a participant in his MS trial who would agree to any danger just to stay in. As feared, in the Georgia governor election the pro-care incumbent is defeated by a businessman who promises heavy welfare cuts. Unprovoked, young stud Stewart Mayer starts a fight at the nearest polling station, causing him, the laborer who opposed him and multiple bystander victims to arrive at Chastain. Recognizing Stewart from his tattoo as a fellow veteran, who however never saw field action, Conrad successfully searches for a medical cause for his aggression: miss-matched medication. Laborer Enrique Mendez is reluctant to 'loose time' for treatment, despite injured ribs, but is diagnosed terminal. Dr. Ian Sullivan is duly annoyed as daughter Kincaid keeps meddling in his consultation as if he were the child, but his prospects are great, while Kit must admit she's terrified for Bell's unpredictable MS, hence inclined to treatment in too many cases. Leela shows AJ that Padma's panic can best be handled by going along in her superstitions when harmless.

After sending a well-insured patient to the -private- nearest hospital on ambulance patrol, Conrad rushes to an anonymous fresh gun wound victim thrown from a speeding car, which gets the code-name Eucalyptus, so fifth already in a record time, refused by the nearest hospital's triage, officially 'full', probably because such patients are usually destitute budget drains, as occurs involving over a hundred health workers, piles of medical equipment and all the available AB-blood. The last proves another complication for Padma giving precipitous birth to her twin with a uterus bleed, panicking while nobody can stay with her constantly. Hearing from Cade that Ian Sullivan is probably addicted, Conrad 'suggests' another drug test, which angers her but he circumvents by hastily taking a back pain medication to explain his alert result, giving him a week to actually get off.

Devon hesitates how far to search for an alternative diagnosis as a myasthenia-gravis patient doesn't respond to treatment at all, finally regaining her mobility. It's drug test day for Ian, who has a full surgery schedule and must wait for the hired private firm to arrive at 16 PM, then is torn away for an urgent operation, all the time bugged by Cade and Billie while he fights off near-disabling detox symptoms. Conrad takes the day off, to train Gigi with anxiety for her taekwondo yellow band test.

Devon politely listens to Marco, who wants scientists to coach his life research for 'eternal life', till he collapses demonstrating his amazing health for a seventy-plus, then works out what 'wonder cures' backfired and gets a surprising long-term reward. ER doc Irving Feldman enjoyed helping Devon, but decides against switching to a specialism where caregivers don't stay so anonymous. Conrad is eager to help college professor Raja Khoury, the first patient he saved as an intern by contrary diagnosis, but soon discovers he missed check ups after cancer remission for years and avoided basic care despite a steel stint in his spine, which now gives major complications. Leela and AJ duly worry as Padma can't cope with her noisy twins' needs.

The mother is found a major cardiac risk, so she must first be patched up so she can donate, two high-risks operations in a row.

After their wedding, newly-elected governor Mark Betz invites Randolph and Kit for what they believed a private discussion on his planned health care budget cuts, but it turns out a media circus. As he repeats his electoral claim that tax money is better spent on crime fighting, Bell can't resist pointing out Betz's own criminal past with conviction of his company for abduction Medicare funds to his hospital chain. Meanwhile Conrad accompanies to jail Raptor, who wants to help Bobby Purcell, the brother on death row of his late mother's palliative nurse. The negligent prison infirmary ignored what proves probably fatal, so he refuses therapy before the lethal injection, until his sister's DNA risk test yields a surprisingly relevant diagnosis. The governor meanwhile blackmails ex-con nurse Donald Killian to spy on Randolph, to publicly accuse him of fatal malpractice.

After a schoolboy is rushed to Chastaine for an overdose by his girlfriend, Conrad realizes it's actually poisonous pills distributed by his big brother, so rushes to their high school just in time for him, but it's close and a schoolmate already perished. Bell's hospital lawyer rightly warns this suit by ruthless shark attorney David Costican is not as easy as just being right. After several testimonies, he must promise Kit to settle as Conrad's pa fears Chastaine may be ruined by a loss, but facing vindictive governor Mark Betz she promises a fight he'll lose, despite his mole orderly Donald Killian. Nearly caught stealing medication, Ian wants Cade to help him kick off, she blackmails him to go to a clinic if she covers up even to Conrad.

No Pressure No Diamonds

Devon scores a dream contract with a tech company for Chastaine to be the first to use its revolutionary titanium substitute ribs, and convinces Kit to give him last say in an arrangement to have that filmed as publicity for both, but patient Tessa Perez gets complications and her daughter only 'allows' it if Bell is replaced in the team, given a social media smear campaign from the governor, by AJ. The stress contributes to Randolph's MS reaching alarm level again after he helps Conrad painstakingly diagnoses an illegal zoo co-owner, brought in by his business partner for 'cat scratches', actually from a lion, but the real danger is poison from their small critters collection.

Conrad and Gigi finally face Nic's bitterly estranged father Kyle Nevin, who is brought despite protest to Chastain with an alarmingly low heart rate; Devon's bedside manner and combined diagnostic genius with Conrad, plus Gigi's grandpa-love, win him over to reconcile, while his last-minute re-diagnosis enables optimal therapy and prospects. Dr. Ian Sullivan charmed the entire rehab clinic, until Kincaid smashes his reputation at family day. Billie advises Doris Chambers against risky surgery, but her redneck sons Lance and Fritz first demand it unconditionally, then swear physical vengeance when a complication kills her. Dr. James Yamada politely near-clashes with AJ, until informed of Cade and Billie's confusing love-life. Padma goes missing, leaving the twins with AJ and hospital daycare.

The governor's discretionary budget cuts require closing down whole sections, but last minute Kit decides to keep ER open, for which many patients have no realistic alternative. The team is excited by the arrival of cardiac crisis patient Dr. Leonard Perry, who keeps self-diagnosing and instructing how to treat him, being an innovative expert and professor in cardiology, who asks for his favorite former student James Yamada, who sides with AJ advising against Perry's signature surgery, but it's pulled off at request, only to lead to a fatal complication. An even bossier patient was purse-robbed but is pulled trough. After Cade confesses her secrecy about pa Ian to Conrad, he comes clean about his kiss with Billie, and is instantly dumped, an awkward way to clear to path for him and Billie as well as James and Cade. Padma nearly commits suicide as bridge jumper but Leela steers her home and to postnatal depression therapy.

All the Wiser

Devon reconsiders his refusal to do the tycoon's longevity research so as to get his own projects financed, as Bell hoping for a second MS trial stage illustrates Leela's point the greater good primes. Dr. Ian Sullivan is out of rehab, but resigns 'for personal reasons' rather than confiding his addiction into Kitt. Governor Mark Betz and his Afghanistan veteran helicopter pilot are brought in despite his objection to Chastain, as it crashed two streets away and other hospitals are overburdened due to ward closures resulting from his budget cuts. Diagnosed with major cardiac troubles requiring a transplant, Betz get Conrad and AJ to give him unnecessary surgery just to jump the queue, in exchange for returning Chastain's budget.

All Hands on Deck

Sammie is rushed to Chastain deathly ill, so Randolph convinces Ian to return midst concerns that her cancer has returned while Betz experiences problems after his heart transplant. By observance, Sammie is diagnosed with Kawasaki disease but also gets a coronary artery aneurysm requiring coronary bypass surgery. Betz is diagnosed with an ischemic bowel, requiring Bell to operate on him; he experiences an MS flare up, but Leela takes over, deciding him to 'pas the torch' as chief surgeon. Ian confides into Kit and Bell his drug addiction, but retains his privileges, being uniquely capable, accepting zero tolerance drug test; Ian and AJ save Sammie. A week later, Betz keeps his word and publicly announces restores Chastain's funding, saving the hospital. Devon confirming Bell's progressed MS makes him eligible for the second phase of Devon's clinical trial, Bell takes a leave of absence and focuses on teaching. Padma is happily reunited with the twins, AJ and Leela promise full support. Leela becomes an attending and accepts Devon's marriage proposal.
