Conrad works on balancing life as a new father without Nick while Devon and lover Leela discretely debate taking the next step in their relationship. With systems closing down, the staff must improvise urgent procedures and delay the rest while diverting new patients. After waiting for hours for his pain medication after a gruesome shoulder reset, Devon's patient wanders off and faints from poisonous fumes, which also floor Devon when he comes looking for the missing man after his shift.

Conrad rushes after Nic to Chastain following her car crash where she incurred a major brain injury from the impact. As the devastated staff mobilizes desperate to save her, Nic's condition proves hopeless while Conrad persists in denial and must be held back 'an an ordinary husband'. Long COVID-19 patient Gabe Sandoval returns with his trachea so weakened from the long intubation on IC that he will have to spend the rest of his life on a ventilator. After Nic's brain herniates, an injury one can't recover from, Devon is forced to declare her brain dead with the help of Hundley and Jessica while a grief-stricken Kyle blames Conrad for not saving his daughter. Respecting Nic's wishes, Conrad agrees to allow her organs to be harvested for transplant before Nic is taken off of life support and dies. Fearing more intubation, Gabe refuses further treatment despite his obsessionally devoted now-nurse wife Cherry's attempts to convince him otherwise. AJ and Leela finally convince Gabe to take a rare trachea transplant instead, one of some eighteen fresh organs from Nic with Conrad's blessing.

Devon and Zach Brooks discover her massively over-medicated with toxic incompatibilities, and that her underlying condition masked by her medications that requires surgery; they diagnose Gloria with Whipple's disease, but she already died in surgery. Devon points out that Zach is the only doctor who had recognized that there was a bigger picture and suggests that he has a future in geriatrics. Gigi urges Conrad to cure her daycare playmate Peter who won't eat, apparently part of abandonment anxiety since his mother left him and devoted daddy Chastain ITC man Spencer Shafer. The boy seems deathly ill, alarming Conrad and Billie when it begins to look like the six year old Peter will need a craniectomy, which she sets up; last minute Conrad realizes from Gigi's sandwich switch observations that Peter has a rare condition where his body can't process proteins which has been worsened by the protein shake that he had shared with his father. Dialysis clears up the problem just in time to avoid the craniectomy. Padma considers an anonymous sperm donor until Leela demonstrates the dangers of it by revealing that Nolan is one; instead, Padma asks AJ to be the father of her child.

Bryce ultimately survives and although the virus damaged his nervous system, can expect a good recovery. After the state medical board's delegate blatantly whitewashes their investigation into rotten doctors, Bell goes public in order to get the doctors taken down, costing him his place on the medical board, calling out the medical board and the governor for their actions in a live talk show interview. Leela and Padma prepare to harvest Leela's eggs, although it is unclear if AJ will be the donor or not; Leela pushes to have the eggs frozen and Devon's suggestion that he be the donor creates tension between him and Leela. AJ stays at his mother's side for days as Carol worsens and approaches the end with AJ struggling to face it; she passes away in her sleep after encouraging AJ to find love and start a family. Ultimately, AJ agrees to donate, but only if he can be a present father rather than an 'anonymous' sperm donor.

Her sister Padma can't handle AJ's plans to almost take over parenting rather then acting as donor plus, even offering his just buried mother's home, so she is delighted when Devon confides he considered donating his perfect sperm, but Leela remains hysterically opposed.

Devon and Leela haven't found a way forward. Notorious anti-medical podcaster Jimmy Miller's respiratory condition requires a long transplant, but the anti-vaccer first refuses to get vaccinated, then confides he sneakily already got all vaccines, infuriating his naive wife Lola.

AJ is greatly impressed by the clever input of child surgeon Dr. Ian Sullivan, who came to rescue Cade with his rare blood, and now considers moving permanently from Seattle and scores an offer from Kit, however Conrad is vehemently against him as Cade took a bad fall opening her stitches, stupidly getting up alone as dad didn't turn up for a walk. Overworked Randolph's MS reaches crisis level, while Ian, who hides his pills dependency, collapses in his hotel room.
