A Wedding, A Funeral

Conrad and Nic had to postpone their unconventional wedding till after the peaks of Covid-19. They and colleagues look back at the traumatic period at Chastaine, when CEO Kim struck a deal with Cain to talk patients out of postponing less urgent surgery as Red Rock wont accept the sharp loss of income. Devon warns his father, in vain, against continuing his exposed job as taxi driver, Kyle leaves rehab at Nic's urging. Nurse Hundley was infected by giving heart massage before proper protection procedures were drilled, Kim only broke the rules and budget to get decent protective equipment after Conrad confronted him.

Mina's Kangaroo Court

Logan Kim is fired as CEO for 'unauthorized use of funds' during the pandemic, but warns Conrad that things will only worsen. AJ and Mina treat a VIP patient whom Cain's lies, as Kim insisted on 'sales quota', tricked during the pandemic to perform an elective surgery, resulting in COVID-19 and blood clots currently. Conrad and Devon treat Congresswoman Nichelle Randall, potentially Georgia's next governor with strange complaints and discover that liar Nichelle and her devoted driver both contracted a parasitic infection in the Caribbean as lovers. Devon still suffers from hallucinations of his late father and confides in Conrad about his death in an understaffed public hospital. Consciously single, Bell reconnects with his bitterly estranged former step-son Jake Phillips/Wong, who became jazz musician and plastic surgeon. AJ, secret lover Mina and Cain save the patient, but AJ, believing that 'black brother' Cain can be redeemed, refuses to report him. Mina alone informs the patient's wife and she and AJ to take a break for a few days. Kit Voss tracks down the chairman of Red Rock who reveals that no new CEO will replace Logan Kim, without elaborating. Nic tells Conrad she's pregnant.

The Accidental Patient

Saving young Eva's life at a massive accident, Cain is hit by an ambulance and severely damaged hands compromise the surgeon's career. Two of his residents staring the surgery as he expected, before elected Kit can arrive, cause more damage; she and AJ save Cain's life, but fear severe brain damage if he will ever regain consciousness at all. Still mourning, Devon attempts to convince reluctant terminal patient Winston to call his estranged daughter; after Winston dies, Devon meets his daughter who thanks him as Winston called to reconcile with her thanks to Devon. Bell decides to televise his surgery on social media star Emmett Mackey, which Mina scolds as exploiting their patient; the routine surgery suddenly turns complicated, but they save Emmett's life on air. Eva is in a Witness Protection Program but ex-boyfriend Darren, whom she left behind, shows up having recognized Eva on TV. Darren helps Conrad and Nic figure out Eva's her chronic illness and he decides to join Eva in Witness Protection for love. After realizing Bell learned that Red Rock intends to sell the hospital and whoever buys it could be worse, so they need good publicity, Mina offers to help and informs AJ. Mina and AJ define their work relationship and Mina informs Nic; Conrad and Nic secretly take a blood test confirming her pregnancy.

Moving on and Mother Hens

Devon still struggles with his haunting father's death, complicated by a self-invited visit from his mother Himaya Pravesh , who again tries to arrange a 'match'; after snapping at her and everyone hinting at therapy, Devon admits he might not be alright and they open up to each other about their struggles. Conrad, Nic and Devon treat Trinity, a young woman with confusing symptoms and a meddler mother who has a heart attack, but AJ and Mina successfully treat her while Trinity is diagnosed with a rare but treatable genetic condition; they reconcile. Bell meets Fiona, a TV show who has a psychotic break, applies for a hospital job and stalks him; Bell and Kit find her symptoms caused by a tumor which they remove. Mina faces possible deportation due to an expiring visa and proposes to AJ; after disappointing legal advise, she calls off the wedding but admits being in love with AJ which he reciprocates. Conrad and Nic discover that they are expecting a girl and spread the news to their friends. Bell learns that Red Rock has sold Chastain to developers who intend to turn the hospital into luxury condos.

And Nic who is still recuperating is visited by her friend a former nurse practitioner who was let go because she performed an unauthorized surgery and who has a tense relationship with Conrad. When something happens to Nic her friend offers to help and Conrad is not sure if she should. Meanwhile, the hospital looks for a new CEO.

Devon and Kit treat a patient who suffers from sickle cell anemia and encourage her to have a hip replacement to help treat her pain. Meanwhile, The Raptor is pulled away in the middle of an intense surgery, Mina is left to put her skills and finish the surgery on her own and Bell works on reconnecting with his stepson (guest star Conrad Ricamora) after helping him get hired at the hospital.

Jake and gay husband Greg's future child's mother Tina is rushed into the ER with a gallbladder infection, Bell performs an emergency C-section to save both mother and child. Tina's traditional parents successfully threaten to disown her if she allows a homosexual couple to adopt the baby despite Bell's attempts to intercede on their behalf, leaving Jake and Greg childless and heartbroken. Devon's old sickle cell patient Rose collapses in physical therapy and requires a spleen removal during which time she cannot have a blood transfusion; Mina and AJ pull off the surgery. With only a week until her visa expires and months for her appeal to be decided, an exhausted Mina chooses to willingly return to Nigeria and AJ decides to come with her. In physical therapy Cain befriends Rose, inspired by her positive attitude and reminded of an old friend of who died of that disease; Feldman and Jessica elope at an aquarium.
