In rural Missouri Jenny Ammory is a young girl who lies compulsively. She neglects to do her homework and when sent to school against her wishes, she meets Richard Kimble - working as a delivery driver under the alias "Jim Fowler" - when she leaves her books by a dirt pathway to consult a rag doll she has named Nayet she keeps near a creek. Kimble/Fowler gives her back her books, she politely thanks him, and both go on. When she arrives at her school, however, she claims Kimble/Fowler attacked her to her teacher Emily Norton. Emily, however, has caught Jenny in lies before, and when Kimble/Fowler delivers some items to the school she talks to him, noting how she is persecuted by the wives of the fathers of her students because of her beauty that has attracted these men to her. When Jenny catches them talking, she is scolded by Emily but runs home and claims Kimble/Fowler and Emily Norton are having an illicit affair. This brings about a confrontation with Kimble/Fowler that threatens Kimble's life and leads to a showdown with Jenny at the creek as a town lynch mob closes in on Kimble.

Dr. Kimble runs from the sheriff after he gets picked up for fighting. He escapes into the mountains of West Virginia where he encounters a young woman, Cassie. Cassie longs to escape herself. There's no future in this former coal mining town. Cassie knows a shortcut through an abandoned mine that cuts through the mountain and comes out on the other side. Cassie spends her days with her elderly grandmother, all the while dreaming of a better life in the city, but she's afraid to go it alone. Cassie asks Kimble to take her to the city with him so he can help her navigate city life, about which she knows nothing. Kimble helps her to find confidence in herself, without needing to lean on a man.

Settled in Santa Barbara as an apprentice sail maker, Dr. Kimble falls for Karen, the niece of his boss. They become romantically involved. There's a lot that keeps him here. His boss likes him, Karen loves him, and even Karen's old flame likes him. The only one who resents his presence is Eric, another worker who was brought up by the boss since age 12. But Gerard and Kimble cross paths in Los Angeles, and while Kimble was able to elude him, he knows that Gerard is on his trail. He confides In Karen, and wonders whether he should keep running, or try to fool Gerard by staying in one place.

The next day, Joe's manager Lou hires Kimble to be his new cut man. Kimble goes to training camp with Joe and his wife, Lou, and others. Joe's wife Laura tells Kimble about some memory loss that Joe had experienced. Kimble fears the onset of brain damage and Joe fears it too because Joe once attended medical school and knows the symptoms. Meanwhile, Lou is being investigated by the boxing commission and because of that, everyone around Lou is also being checked out, including Kimble.

A number of them are Mexican laborers who are suspicious about his presence - after all, he doesn't look like one of them. The workers are recruited to help fight a forest fire. When a pregnant woman starts to feel labor pains, the nurse discovers she can't have a natural delivery. They need an obstetrician to perform a cesarean section. But with fire all around them, and the roads blocked, it's impossible to get to a hospital in time.

The young murderers are headed to Hollywood, to fry bigger fish.

A man comes on board while the ship is in transit and reveals to the Captain he's a cop who's going to arrest one of the passengers but must first be sure. He is placed with Kimble. When the man is killed, the Captain has to find out who the killer is before they dock. And when Kimble is the last man who was with him before he was found, that they know of. And when the Captain has him checked out and tells him that they discovered that his identity is false, he tells Kimble that when they dock he will turn him over to the authorities. So Kimble has to find out who the killer is before then.

Gerard arrives, Kimble breaks out of the jail with a friendly overweight young man. Kimble witnesses his boss, Martin Rowland, punching out a man in a club parking lot. After Mr. Rowland leaves, the man collapses and dies. Kimble phones the police to report the accident and when the police get there, they arrest an old transient who was hiding nearby with a stolen watch and wallet. Rowland and Kimble have a mutual respect, but Kimble keeps trying to anonymously push Rowland to confess to the fight and clear the old transient of the murder charge. Rowland actually does want to "do the right thing" but Rowland's wife doesn't care that the innocent old man might go to prison for murder because she knows that the fight was over her and she doesn't want any negative publicity.

Dr. Kimble is now working as a "timekeeper" at a Utah construction site. There's also a young man named Jamie working there as a "water boy" whom the heavy machinery operators make fun of and generally try to run out. They succeed. The boss fires Jamie, but Kimble tells Jamie that he should go say goodbye to the boss's wife before he leaves in the morning. That night, the boss's wife is attacked and Jamie is witnessed leaving her trailer immediately after the attack. Kimble tries to convince the boss and everyone else that they're accusing the wrong man.

Once Carol, a photographer, discovers who Kimble is, the plan changes so that Ann will be murdered but it'll look like Kimble did it. Kimble tries to warn Ann, but she doesn't believe him because she loves her husband and sister.

The father suspects Kimble is trysting by the pool with the much younger, flirty Chris, which could lead to louder hush money, which neither the casino nor the convicted killer want.

The fugitive's struggling to keep a low profile, but his principles won't let him shoot the runaway bitch because she appears readily savable, though Cora icily orders Kimble to kill the Irish Setter. The scared dog was injured on the estate's barbed wire, used to keep thieves out, not the highly-trained canines in. This live-in job provides a safe hiding place for the fugitive physician, who worries what Cora's up to.

The fugitive's liquor shop gets hit, but Kimble's boss plugs this easily-foiled robber. The inept Herbie's too reluctant to shoot, to be the wanted man. He's hiding in the store's vehicle when Kimble jumps in to make a delivery. Kimble patches up his fellow fugitive's light wound and gets Herbie to set out on foot. Kimble's concerned that testifying against Herbie will reveal the wanted physician's true identity, plus get the small-time hood hung for murders he didn't commit, as Kimble was. But Herbie the rat sticks to the inventive Kimble like a parasite.

On his way out of town after finishing a stint as a migrant farm worker, Dr. Kimble is prevailed upon to help a couple who claims their son needs a doctor. Though Kimble says he's not a doctor, that he just knows first aid, the couple are desperate. No one else is around who can help. Once Kimble arrives at the cabin, he finds out that things are not as they were presented to him. It turns out that the child is a kidnap victim who sustained some bruising when he was snatched. As a hemophiliac, there's a danger of internal bleeding. When Kimble can't convince the kidnappers to take the child to a hospital, he asks them to go to a pharmacy for vitamin K, a coagulant that would promote blood clotting. But the pharmacy won't dispense it without a prescription. Kimble must come up with a way to save the child.

Five years, prior, Marcie had gone to Dr. Kimble's office seeking an abortion. Since it was illegal, he refused. She got the abortion from another place, but was left without the ability to have future children. Because of that, Marcie holds a grudge against Dr. Kimble. When Marcie sees Dr. Kimble in Florida, she subtly threatens to expose him unless he takes her and her boyfriend out to her house in the Florida Keys during the middle of a hurricane. The boyfriend gets swept overboard, but Dr. Kimble and Marcie make it to the house, where they spend an eventful night. During the night, to gain Dr. Kimble's sympathy, Marcie pretends to be someone she's not. They eventually come to a kind of truce. But Marcie can't help her scheming, and radios the Coast Guard to get word to the local sheriff that Dr. Richard Kimble is hiding out with her. Dr. Kimble finds out. Marcie has a change of heart and helps hatch a plan to help Dr. Kimble escape capture.
