Lotte and Jacob have differences of opinion about their relationship. In the operating room, Lotte as the surgeon is the boss although Jacob as the anesthesiologist likes to think they are equals in doing different jobs. At home, Lotte proverbially wears the pants in the house, although Jacob doesn't like to think so. Their social world is shifting with Ben now starting to date much younger Alyssa, with Gemma still in their professional and personal lives, the latter as Lotte's best girlfriend. However professionally outside the operating room, Lotte and Jacob only associate with fellow doctors and administrators, the nurses too understanding under head nurse Annette's iron hand. Things change with the arrival onto the scene at Leeds Hospital of scrub nurse Mia from her native Norway. Mia breaks that protocol by insinuating herself with whoever she pleases, she taking it one step further in renting the house in the cul-de-sac next to Lotte and Jacob. Lotte and Jacob's lives are potentially further complicated with the arrival back to Leeds of Lotte's old boyfriend Leo, come home to deal with his dying father who has been admitted to the hospital. Lotte may need that extra emotional support in they dealing with Henry's declining cognitive abilities.

The proverbial morning after the drunken evening, Lotte feels the need to set some boundaries with Mia without hurting her feelings, Mia in return seemingly taking the news as intended. A series of incidents leads to an investigation at the hospital centered on Mia and a patient. And Leo has returned to Leeds to find that his family's foundation is sponsoring the new wing of the hospital, he who wants to be involved partly in not trusting Hari, the new head of the foundation in their father's illness. Things with all the players come to a head at the foundation's fundraising ball, where Mia takes charge for her own end goals.

Beyond the Mia's initiated sexual indiscretion following the ball, Jacob and Mia's lives are becoming more entwined when she catches him in one of his secret stress reducing pastimes at the hospital. Perhaps partly to alleviate his guilt, Jacob surprises Lotte with what is meant to be a romantic twentieth anniversary getaway to Antwerp, it to mirror their first ever romantic trip as a couple. The romance part of the trip hits some rough patches for both Lotte and Jacob partly for the same reason unknown to the other. Back at the hospital, Ben thinks about moving to the next step of the investigation into the incident involving Mr. Green, a former patient. Gemma feels the need to play brother off of brother when she receives a threat from Hari. And a lonely Alan contemplates reentering the dating scene, he knowing his past an impediment.

Lotte, Jacob and Mia have returned from Antwerp, each with a different rearview mirror perspective of what happened and what they would like to happen moving forward. Things between Jacob and Mia are only complicated by work issues, in Mia knowing about Jacob's "pharmaceutical" secret, but she not knowing that he has been tasked with doing the groundwork in the staffing issues in the investigation regarding Mr. Green's death. It is also complicated by much of the hospital staff, especially the nursing staff, seeing that there is something going on specifically between Jacob and Mia. Mia also returns to discover something that she feared would happen which may bring up her hidden past. And at the neighborhood bonfire and fireworks, an old and a new connection are made.

Jacob confronts Lotte in his hope to fix what is wrong in their marriage, Lotte who may see his accusations a bit differently. His findings into the Ben directed investigations at the hospital take a turn with yet another suspicious death of a patient, this one a little more high profile. Jacob's information may have the intended effect of taking down Mia, but it may also come back to bite him as not all the pieces add up. One of his actions also reunites Mia with Isak. And despite knowing his history, Yvonne continues her pursuit of Alan.

Isak has reinserted himself in Mia's life, which Jacob sees as a good thing in more ways than one, although Alan doesn't trust this strange new man in Mia's life. Jacob is unaware that he is the new focus of Gemma and Leo's investigation specifically into the death of Leo's father, Lotte who is assisting them in being the one who can provide an alibi for the previous suspect, Mia. Lotte has to decide how much of a risk she is willing to take if Jacob indeed did purposefully kill and as their marriage quickly starts to fall apart because of his recent secrets.
