They possess a little girl, two swimmers from a high school team and a demolition worker and they are hungry. Meanwhile Sam confesses to Dean and Bobby that he has difficulties to identify what is real or hallucination and he sees Lucifer everywhere. The girl goes to the Sioux Falls General Hospital and possesses the surgeon Dr. Gaines. However Sheriff Jody Mills, who is recovering from an appendectomy, witnesses the leviathan eating the liver of her roommate and she calls Bobby. Meanwhile, the four leviathans meet at the hospital and they eat the head nurse and one administrator. Bobby goes to the hospital and rescues Sheriff Mills. Meanwhile Sam is lured by Lucifer posing as Dean. When the real Dean comes to meet Sam, he needs to know how to find out what is real.

However he discovers that Amy and her mother are Kitsunes that feed with human brains. When her mother is ready to kill him, he is protected by Amy. Now Amy explains why she killed her victims and Sam let her go. Now he has to convince Dean to spare Amy.

Osiris decides to judge Dean's guilt and Sam becomes his defense lawyer. However when Osiris is ready to summon his last witness, Dean realizes that he will summon Jo and he takes the blame.

Furthermore, they discover that Maggie is a powerful witch and Don is a powerful warlock. Meanwhile, a leviathan is hunting down the Winchester brothers and waiting for them in their hotel room.

Furthermore, they are the second most wanted in the United States, and they are the object of an FBI manhunt due to the murders committed by their doppelgangers. Dean and Sam head to Ankeny, Iowa, and when they see the doppelgangers, Sheriff Osborne and his men arrest the Winchester brothers. Dean asks the sheriff to use his right of a phone call. Meanwhile, Sheriff Jody Mills visits Bobby who is still trying to destroy Chet. Out of the blue, a liquid drops on Chet arm and melts his flesh. Bobby finds Jody cleaning the floor with a sodium borate based detergent and learns that decapitation associated with sodium borate can destroy the evil leviathans.

When Bobby is captured by four Leviathans, Dean and Sam question how they can rescue Bobby.

Sam meets Krissy and confirms that her father is a hunter called Lee Chambers who is missing. Sam finds a map and information hidden in Lee's room showing that he was chasing a Vetala. Sam heads to the spot, expecting to find one Vetala arrives in a truck stop, but, instead, they are in two, and they is capture Sam. Meanwhile, Frank finds that the numbers are a coordinate to land which belongs to Dick. While Frank keeps the place under surveillance, Dean goes to Krissy's house to seek out Sam, but the girl destroyed the map and information, and Dean is forced to travel with her to seek out Sam, Lee and the dangerous Vetalas.

They stalk him, and, when Mr. Snider is attacking another victim, Dean jumps over him, and they vanish in a red light. Dean is sent back to 1944 and is arrested by the police, but Eliot Ness releases him and teams-up with Dean to chase Mr. Snider, who is actually Chronus, the Time God. Meanwhile, Sam and Jody are trying to find a means to bring Dean back to 2012.

On the next morning, Dean goes to Lydia's place to get Bobby's flask that he forgot there and he witnesses Lydia talking to her baby daughter Emma. Dean decides to keep them under surveillance. Meanwhile Sam discovers that the symbol is relative to Amazon warriors and they can reproduce in 36 hours. Soon Dean receives Emma in his room and learns that she is his daughter while Sam finds that all the victims were killed by their own daughters.

Meanwhile Sam, who is seeing Lucifer everywhere, visits Nora and discovers a secret about the murders.

Soon the angels Hester and Inias appear and tell that Kevin is the prophet of God. However Dean uses a symbol to get rid off the angels to give time to Kevin to translate the tablet. When two angels bring Kevin back home, there is a surprise waiting for them.
