With Janeway's approval, Paris enters the Delta Flyer in an inter-species, light-speed shuttle race. Four races, once at war, hold the race as a celebration of their peace treaty. Wrapped up in competition, Paris forgets a promise to spend a weekend with B'Elanna. Torres is hurt but, with advice from Seven (of all people), she decides to partake in the race as Paris' co-pilot. When the race is underway, the couple realizes their participation was intentional: the Delta Flyer has been rigged as a bomb by one of the racers seeking to end the peace accord with a bang.

Chief of Security Tuvok, investigating alongside Harry and Tom, finds the attacks strangely illogical. The culprit's identity takes everyone by surprise, even Tuvok, with the goal nothing less than the divisive restoration of an old agenda.

The Doctor is stolen from Voyager by an alien who sells him and his mobile emitter to a hospital on a densely-populated planet. He is activated in a cramped room filled with sick people and is made to treat them using only very basic equipment. When the administrators of the facility realise how skilled the Doctor is, they move him to another area of the hospital to treat more deserving patients. The Doctor is appalled to learn that the medical care that people are given is linked to their status in society and hatches a plan to make things a little less discriminatory.

Turns out that wariness is good as the transmitted program has been compromised.

Meanwhile, Engineer Donik (the Hirogen "coward") assists Janeway aboard the damaged Voyager to retrieve the captive crewmen while trying to thwart two Hirogen hunting parties.

Now newly wracked with guilt - a punishment in itself - he welcomes his coming execution for his crimes, for which Captain Janeway argues leniency and reconsideration to his captors. Meanwhile, Neelix, through a second prisoner, learns of the partiality of the Nygean justice system.

Janeway sends some men to retrieve it but they discover that the planet it is on is inhabited, and the inhabitants believe that the probe is a weapon because it emanates an energy that is harming them. They tell Janeway to help them relocate. But Seven thinks they can cure them and remove the energy that is harming them. And when Janeway tells them, they don't believe her. They insist that Janeway relocate them and if she doesn't they will kill her people.

Tuvok, Neelix and Tom investigate aboard the Delta Flyer, run afoul of explosive mining operations, and end up as temporary prisoners of the colony. Since they are of his race, Neelix helps the colonists take a more assertive stance against the asteroid miners threatening their home (rather than follow their usual flee-and-survive tactic), which leads Neelix to consider a more personal decision.
