Set in Riverside on the upper West Side of Manhattan, Ryan's Hope centered mostly on the working class Ryans, an Irish-American family headed by Maeve and Johnny Ryan. They owned Ryan's Bar, a pivotal location for character interaction. The Ryans' dreams and ambitions made up most of the stories.

At Ryan's Bar on the west side of Manhattan, the Ryan family prepares a celebration for eldest son Frank, a former NYPD cop who is now an attorney and running for city council. While his beloved sister Mary wonders why he's late, across the street Frank tumbles down the back stairs of a Riverside Hospital stairwell. Later, Mary meets prickly newspaper reporter Jack Fenelli who isn't a fan of the Ryans. Mary and Frank's brother Patrick Ryan, a doctor at Riverside, enthralls fellow resident Faith Coleridge with an Irish tale. Their mutual friend Bucky carter, also a resident, is called on the carpet by the hospital's hard charge nurse Ramona Gonzales. Ryan family matriarch Maeve Ryan comforts daughter-in-law Delia, Frank's wife, who claims that she's shaken after nearly being hit by a taxi in the street. Meanwhile, Frank lies unconscious the bottom of the stairs.
