Evan and Hank can only agree on having totally different views on HankMed, so their paths will separate on Monday. First they must attend the annual harbor feast, a previous obligation. Notably the eating contest yields interesting patients for Hank and Divya, who learns that her former fiancée is setting a date for her successor. Evan hires Paul, but he can only commit part-time given his hospital job, hence also somewhat freakish Hank-lookalike Jeremiah Sacani.

Evan and Hank find dividing the HankMed assets easy, a brotherly relief helping to swear it won't wreck their family bond, but the patients and Divya are a different matter. Brilliant nerd Luke turns up at Aunt Jill's door, claiming to be expelled from chess camp for fighting, but actually suffers from a bully and a psychosomatic problem, while her third-world job prospects fall through after she's sold her house and quit her hospital job. Evan misses Hank's medical genius, his brother's business nose and managerial skill, as well as capital to start his own concierge doctor practice.

Evan and Hank are now "friendly" business-rivals, a competition which Divya hopes to sneakily end by working for both--and actually sabotaging each. Evan now finds himself overstretched as he also has to work in socially-awkward doc Sacani, so lacks time to pay proper attention to suddenly-returned Paige. Meanwhile, a suspicious number of atypical pneumonia cases starts pointing to a luxury spa they both rivaled for as ideal institutional client and patients recruiting ground. Hank hopes that Jill can help him with the business side, but she's offered an even-better dream job and starts packing to leave at once.

Hank finally acknowledges Evan's brilliant contribution on the business side, and the brothers agree to team up again in HankMed 3.0, with Ken Keller as "silent" partner, who still needs to grasp that adjective. Its first staff meeting going ape, Evan decides to limit those to the brothers and Divya, whose shares are vote-neutralized. Hank treats a matchmaker and a shot intruder in Boris' castle. Later, father Eddie reports the cloak-and-dagger events with fake "mover trucks" on that evening and the intruder's mysterious removal scare Evan and irritate Hank enough to demand the truth, an attempt at Boris's life due to his greedy family feud. Only after Evan realizes that Paige's obsessive need to be around and help as perfect host requires his full attention, justified as she admits having been fired because the London director only hired her as stage artist out of "casting-couch" interest.

Paul being unavailable, Divya is sent to fainting veterinarian Dr. Tom Fanci, who blames his condition on exhaustion and his workaholism on his vicious ex's divorce lawyer, but Hank must be called in and finds more medical factors. Hank enjoys Boris' bonus: a luxury night with foxy Russan interpreter Christina DuMond.

Although grumpy after Boris warns Hank against romantic expectations with Russian translator Christina DuMond, he duly reports that his playmate may have spied, which gets her fired and a loss for Hank, who then turns in his pager. Jeremy requests social coaching from Divya, but learns quickly, as he proves with martial arts star and new father JJ Small, whose wife fears that his excessively emotional "postnatal" state compromises his career.
