Dr. Sam Stewart runs a medical clinic on Hope Island and she also collects venomous sea snakes to assisting in the making of medicines such as ointments to cure ulcers. A new doctor arrives on the island, although his introduction becomes a bit of a revelation to him. A scuba diver, frightened by a shark, comes up too quickly and suffers from the bends. However, a decompression chamber is too far away to save him.

Rick is making a few waves with the staff as he settles into the practice. A honeymoon couple are staying on the island but the bride is not used to the outback life. Sonny comes up with the solution. When the bride steps on a poisonous stonefish the whole staff become on edge as Sam and Rick disagree on the treatment. It's not helped by a hysterical bride, who also gets some unexpected information. Jack is still unwilling to speak to his father, which concerns Sam. Sam and Gus manage to come up with a sustainable way to milk the stonefish venom.

Sam goes to the mainland to give a lecture to medical students on tropical medicine to help pay for a new outboard for the runabout. Andrew continues trying to convince Sam to transfer to his university but the rivalry continues. Sam is still trying to get Justin to communicate with his father but he is reluctant to do it. A young girl is brought into the clinic going into anaphylactic shock after being bitten by something. Rick goes on a hunt for the cause.

It's Freya's day off so Sam and Gus take her on a hunt for some venomous spiders in order to milk their venom. When Sam plays a practical joke on him, Gus falls into a Gympie stinger bush, and he lets everyone know. After a long process, all the stingers are removed. Sonny then takes Freya snorkelling out onto the reef where he collapses into unconsciousness after suffering a diabetic attack. She manages to drag him back to shore and revive him. Jack has a crush on Freya but has trouble catching her eye.

It's the monthly clinic day on Hope Island, which is a social as well as a medical day for the islanders. Sam and Rick are very busy with the locals coming to get their problems attended to. Rick's curtness and lack of people skills with the patients is putting some of the locals off. A know-it-all amateur herpetologist is caught poaching snake eggs and causes chaos when a baby eastern brown snake is let loose in the clinic.

Sam has a meeting with her Andrew over an upgrade to Hope Clinic. However, it is not as good as she had hoped for with the money coming from a research foundation. Sam meets the foundation's rep but her temper almost ruins the meeting. Rick had a heavy night at the karaoke bar and can't remember a woman he met there. Nevertheless, she wants to continue their night but he sends them to a butterfly grotto, which is guarded by a python. Freya helps the 'romance' along. Nell has been lent a temporary tattoo kit and she practices her skills on Jack. Unfortunately, the tattoos are not very temporary.

Nobody knows that Toby is diving alone and he runs into some trouble with sharks. He activates his PLB so Gus, Sam and Jack go looking for him. They find him but he is trapped under dozens of sharks. Jack's quick thinking allows him to come up but he is suffering from signs of shock and Sam doesn't know why. Why he was recklessly diving alone is also a mystery that Sam is determined to solve. Rick and Freya are hitting it off but she is much more active and adventurous than he is. Gus and Olivia are also getting on well together but she is reluctant to date him.

Sam receives the happy news that they are going to be funded to run a full trial of her new venom ulcer cream. However, she is not happy that a condition of the funding is that Malcolm is to conduct the trial on her behalf. Sam fears that he will manipulate the results to suit himself. However, Malcolm is managing to charm Livi during his extended stay on the island. In a forlorn attempt to put him off, Sam and Gus decide to take out on the water catching sea snakes. It takes more than this to make him leave Hope Island. One of Rick's patients self-diagnoses himself and decides that he has a brain tumour. A patient of Sam's is causing problems for her carer with her imagination.

Two old friends of Sam and Jack have come to Hope Island. They are a husband and wife team of wildlife documentary makers. During some filming on the reef, the anchor of their boat comes lose and the tide takes it away from them. When the boat is found empty and adrift, a search and rescue mission is mounted. Not knowing where they had dived meant that it was going to be a very long search. Meanwhile, one of the local police officers has come down seriously ill with an abnormally fast heart rate. The exact remedy causes Rick some anxious moments.

On the anniversary of her father's death, Sam is going to scatter her father's ashes in the sea surrounded by family and friends. Sam and Jack are on edge when her ex-husband Andrew arrives with his new girlfriend, Gillian, and her daughter, Phoebe. Phoebe also has a chip on her shoulder about the visit. However, things warm up between Jack and Phoebe when they go for a swim at a local waterfall. Gillian also has a secret. An eastern brown snake is feeling sick and the reason has the staff at the clinic stumped. It appears to be dead when it unexpectedly bites Nell.

A storm hits Hope Island leaving a path of destruction. The next day is spent mopping up. It turns out that the local IT expert and satellite installer is an old friend of Rick's, and Freya is intrigued by the influence he has on Rick. His devil-may-care attitude leads to a serious accident where his true colours are shown. A man appears after the storm and helps Sonny with the clean-up. His mysterious past has everybody intrigued, especially when he seems very interested in Sonny's past. He comes with some surprising news.

Toby finds a bell from the wreck of the Esperança and Erica manages to sell it for a lot of money. The dilemma is whether to publicly declare that he has found a wreck and share the money with the government. Nell buys a metal detector and uses it to look for lost coins and other valuables. A similar predicament to Toby's occurs when she finds a lost ring. A father, who has multiple sclerosis, is supporting his daughter with her swimming training when she is accidentally hit by a kayak. She is seriously injured with a tear on her lung.

It's the Irukandji jellyfish season, which are locally known as stingers. This is bad for business as the tourist leave as they can no longer swim in the surrounding sea. Sam uses her father's notes to help find where she thinks is they are breeding. During a night dive, she manages to collect lots of them for her research. However, when they are being milked for their toxin, Jack is accidentally stung. As there is no antivenom, he is in for a very agonising time as all they can do is try and alleviate the pain. It's near the end of Freya's placement on Hope Island. Rick and Freya are feeling miserable over their impending separation. Jack is still suspicious about Toby's motives and thinks he wants to leave. Toby decides to rethink his priorities.
