Nina Proudman is a paediatric doctor, trying to impose order on her life that is daily affected by the chaotic lives of her family. In this pilot episode we meet Chris Havel (Don Hany), prime candidate for a romantic interest. Nina's sister, Billie, attempts 'makeup sex' with boyfriend Mick, but their differences in outlook seem difficult to overcome. Nina's old friend and workmate, Cherie, returns to the hospital for a pregnancy check-up but goes into premature labour. The identity of the baby's father is revealed during the delivery - turning life more complicated for Nina. Welcome to the messy world of Offspring!

Frustrated by the constant accusations, Nina returns to the hospital and reaches a compromise with the staff and the expectant couple, and the nude birth goes smoothly. Later that night an inspired Nina decides to seize the moment with Chris, but is horrified when she interrupts he and Zara in a compromising position in the storeroom. Devastated, Nina rushes home, at a complete loss about where she stands with Dr. Havel. Meanwhile Geraldine decides to try online dating, and Jimmy is an unexpected saviour to Cherie and Darcy.

Cherie and Darcy host a naming ceremony for Baby Ray that sends the Proudman women into a spin. Billie goes to great lengths to make Mick jealous and Jimmy realises that lying has its limits.

Despite having to 'fake it' because they lack physical chemistry, Billie is compelled to persevere with the relationship when she discovers Mick's ex-flame Ivy is back on the scene. Meanwhile, Cherie and Darcy negotiate the rules of dating as Darcy has a life-changing one-night stand.

To her distress Chris is called away to see a patient before she can gauge his response and Nina must turn her focus to a highly risky caesarean on a woman in an induced coma. Still feeling terrible, Nina wonders if she's worthy of such an enormous responsibility. When she finally has a chance to apologise to Chris, he does not pass judgement. And just when Nina and Chris seem to be on track - Dr Havel gets a surprise visitor at the hospital, and all unravels again.

Meanwhile, Billie is confronted by the possibility of motherhood, when she reluctantly accepts the position as Stacey's birthing partner. After the birth, Stacey suggests that Billie could offer her baby the type of life that Stacey could never provide. But is Billie ready to be a parent? Later, Cherie finds it difficult to be upfront with her new boyfriend about baby Ray, and Jimmy gets more than he bargained for when he becomes a human test subject.

Almost as shocking as the act itself, Nina will shatter her sister's new-found loved-up happiness with Mick when she finally brings herself to tell Billie what she did - but not before Mick tells Nina he is going to tell Billie himself and Nina can't stop him. Nina has an 'awkward is an understatement' meeting with Chris' wife Alice, where, unable to stop imagining the fallout from Mick's confession, she cuts her meeting short then inadvertently runs over Alice's foot in her haste to drive off and confront Billie. She rushes Alice to the emergency room, there are more awkward moments with Chris and confusion as to why Nina was with Alice in the first place. Nina blurts out to Alice that she slept with her sister's ex. Mick calls - he couldn't bring himself to do it. Nina must finally go home and face the music with Billie - she must be the one to break the news.
