In a dream sequence, Christian is found bled to death with his throat slit and Sean and Julia make funerary speeches. Sean tells Christian the only way to get over it is to return to work, but Christian is adrift. He can't concentrate, on surgery nor even sex. Julia is happy with her gorgeous, bright younger lover, Jude Sawyer, but he's eager for more, so she asks Sean for a divorce. Christian proposes to Kimber Henry, who rejects him in his non-predatory state. Sean can't keep rescheduling clients, and decides the business needs another partner, so he alone signs a six month contract with Dr. Quentin Costa, and Christian is initially against. Sean agrees to operate on Momma Boone, whose 600 pound weight and crushing fear immobilized her for years on the couch and has made her skin fuse with it. She is septic, and her heart gives way in surgery. At her funeral, Christian convinces Sean to let Julia go. A new investigator of the Carver crimes reveals how she lost her husband, seducing Christian while simultaneously interviewing him. When Kimber surprises them during sex, Christian says he's back and gets her into a threesome.

While business is sky rocketing for Julia, Quentin finally decided the "McNamara and Troy" join with Julia's business. Quentin and Julia also start to have feelings for one another.

The only problem is that Christian believes Quentin is a cocaine addict and is not fit to perform for the surgery. So Christian brings in Sean for extra help. Kimber instructs a patient of Christian's on how to be like Kimber.

Sean finally decides to come back to the practice with Christian.

Quentin seduces the young man while he's recuperating and Sean walks in on them. Sean tries to tell Julia about Quentin having an affair with a man, but Julia chooses not to listen. When Quentin finds out that Sean knew about him and the male patient, Quentin decides to quit his job at McNamara/Troy and to go and work for Julia.

Julia explodes at her mom telling her to take the next flight out and to stay out of her life. Later that day Julia hears that the plane that her mother took crashed right when it was taking off. Julia rushed to the air port to go see if her mom was one of the only few survivors, she quickly called Sean to come and help look and to help perform surgeries on the passengers who were trying to live. Julia also got to experience some surgeries.

They decided that they wanted to give their characters a new thinner image for children to know. When Julia is help putting up Christmas decorations she collapses. Julia goes to Sean to get her blood checked to see if there is anything wrong. She thinks it has to do with the stress of firring Quentin and getting divorced. But Sean discovers later that day, that she is pregnant. While Matt and his girlfriend Ariel are at school they see that their is an African-American nativity scene. Ariel gets very angry and has Matt help her paint the scene white.

Seans asks Julia to move back into the house, but Julia declines his offer. When interrogating Quentin, Kit finds a very shocking fact about him.

In this very shocking and nail biting season finale we find out who "The Carver," really is.
