Following a terrorist attack in London GP Mona Harcourt learns that her brother Kareem has been murdered, apparently by a group called Saif Ali Jasim at his charity clinic in Lahore. For Mona, her siblings and husband Guy there is further distress when Kareem's student son Danish disappears, seemingly to Spain with the knowledge of his uncle Omar. However DS Joe Townsend suspects Danish of terrorist activities and Mona's phone call to her nephew reveals that he is in Lahore.

At the opera Mona and Guy meet DCI Barnes who proposes that Mona fly to Lahore to identify Kareem's body so that it may be released to his family. Accompanied by her mother she travels to Pakistan, leaving Guy and their son Sammi behind. With Kareem gone Mona takes over his work at the clinic and is approached by Richard Payne, the British consul, who takes her to identify Kareem, but she is mistrustful of him and makes a quick exit. Eventually she tracks down suspected terrorist Danish but they are pursued and shots ring out.

Wounded in Lahore by snipers aiming at her and Danish Mona recovers in hospital and is visited by a woman claiming that Danish needs £12,000 for an escape passport and DS Townsend, who denies any involvement in the shooting and releases Kareem's body for his funeral in England. Returning home Mona tells Guy what happened, sending Danish the money but is hauled in by the police, who present evidence linking Danish with Saif Ali Jaz and ask her to testify. Townsend takes Danish into protective custody in Lahore but things do not go well.

Mona is interrogated by the police after the murder of DS Townsend but released and she also learns that her mother financed Danish's trip to Pakistan to see his father, fuelling her suspicion that Danish might be involved with terrorism. In Lahore he is drawn into an extremist group but rings Mona to warn that they are planning another attack in England, entering the country with them. Meanwhile Guy is investigated for his dealings with the mysterious Mr Khalid.

As Mona and Guy try to hold their marriage together the police request that she stays in contact with Danish but Omar innocently intercepts a call from Danish, putting himself in danger as the group attacks him. Danish contrives a situation to introduce Mona to the other cell members but this imperils her when her identity is made known. At the same time Guy's client Mr Khalid is shown to have links to the terrorists' indoctrinator Sheikh Almasi.

As Danish is taken by terrorist Hamoud to carry out their attack the police rescue Mona, reuniting her with Guy but, thanks to Danish, they work out where the bomb will be planted and give chase. Meanwhile Guy helps to prevent Khalid fleeing the country at the cost of his job. Six months later the Shirani family unite to make a statement regarding radicalization in the light of their experiences.
