After recovering from a serious mental breakdown, former top FBI profiler Frank Black, who has a strange gift that lets him see how a killer's mind works, returns to his hometown of Seattle with his wife, Catherine, a clinical social worker who mostly works with victimized children, and their lovable little daughter, Jordan, the two people Frank cares about the most in the world, to work as a consultant for the mysterious Millennium Group, a secretive private organization that hires experts to mostly hunt down serial killers obsessed with doomsday prophecies. Frank's partners include Peter Watts, a friend from the Group who serves as his handler, his old friend, Lt. Bob 'Bletch' Bletcher, from the Seattle Police Department, and sympathetic SPD detective Bob Giebelhouse, who also occasionally asks for his help. In Season 2, he partners up and befriends with the group's latest recruit, Lara Means, a young woman troubled by a gift similar to his. In Season 3, he partners up with the young, dedicated FBI agent Emma Hollis who wants to prove herself to him, which sometimes backfires on her. Most of the cases he works on involve killers, lunatics, conspirators, and amoral people with criminal intent. However, he, who does not believe in the supernatural, must also deal on occasion with those who may or may not be of an actual demonic or angelic nature and not simply evil. His loyalties end up being tested time and time again by both the other-worldly and the Group, which itself seems to be obsessed with the upcoming end of the millennium.

In an airplane with many children, a blonde woman goes to the toilet and takes a hidden pistol inside the towel dispenser. She smokes a cigarette activating the smoke alarm. The flight attendant opens the toilet, takes the pistol from the woman and shots the ceiling with the whole magazine, crashing the airplane. Frank Black is in therapy five months after the death of Catherine Black and moves with Jordan to a new home. He goes to his psychiatrist and tells her that he is anxious to work again. The FBI leads the investigation of the crashed airplane with FAA and NTSB under the command of the ambitious, but talentless Agent Barry Baldwin and when Franks has visions of the plane crash, he asks Assistant Director Andy McClaren to participate of the investigation. He meets Agent Baldwyn and Agent Emma Hollis, and she tells him that the fingerprint of the flight attendant were found on the pistol and Balwyn is blaming her, but Frank finds a cigarette in the lavatory and believes the attendant was not alone. When a house blows up with a woman and a two-year-old girl inside, Frank believes the outbreak, the plane crash and the explosion are related, but Andy does not believe him.
