For the first stunt, contestants on a flatbed truck grab a rope ladder hanging underneath a helicopter and climb it, releasing 4 flags on the ladder. For the second stunt, they are chained to stake with three combination locks and must use their faces to move disgusting items on three tables to reveal the combinations to the locks. After unlocking them, they have to retrieve a pig's foot with their mouth and drop it into a bucket. In the finale, they race a car down a runway and drive it under a moving semi as many times as possible before crashing or running out of road. The contestant to get the most successful passes would win the $50,000.

For the first stunt, the ladies are inside a plastic box and lowered into cold water. With an air pocket at the top, they swim to the bottom, turn two wheels to drop the bottom out of the box, escape, and swim to the surface. The four that do this the fastest would advance. It's reptiles for second stunt. Going two a time, they are seated in a Plexiglas box of boas, pythons, and geckos. While in the box, they have to use metal rods to transfer magnetic disks from slots in the bottom of the box to slots in the top of the box. They lady in each race to transfer 5 magnets first advance to the finals. In the finale, the ladies have to walk through a Plexiglas wind tunnel and collect flags as a fan blew wind and water through the tunnel at 100 mph. As time went on, the tunnel would tilt steeper and steeper. The lady to collect the most flags the fastest before sliding out of the tunnel and plunging into the water below would be the Female Fear Factor Champion and win 50K.

In the first stunt, contestants are inside a Plexiglas box over 100 feet in the air. They have two minutes to wedge themselves in the box before the bottom dropped out. The box would spin around and cold water would rain on them. The two men and the two women who could stay wedged inside the box the longest would advance. Escargot to go as in the second eating stunt they have to eat 12 live snails. The man and the woman who could eat their snails the fastest would advance. In the final, contestants are seated in a car inside the back of a moving semi truck. When a green light flashed, they would have to find which of three keys started the car, and crash it though the back door of the trailer to the pavement. The contestant to get their car the closest to the starting line would win the $50,000.

This was a special female models episode. In the first stunt, the models are chained into a weighted vest and dropped underwater. They have to undo three sets of clasps to get out of the vest and swim to the surface. For the second stunt, the models have to go through an obstacle course in a swamp and find three keys. They would then swim to shore and find out which of the keys unlocked a tunnel. They have to crawl inside and pull an 8-foot long, 400 pound alligator out by its tail. During the finale, the models navigate a series of ropes hanging underneath a helicopter. As they made their way from rope to rope, they would have to release flags attached to the ropes. The lady to release the most flags before falling into the water below would be the Models Fear Factor Champion and will 50K.

For the first stunt, the ladies are locked inside a tumbler lowered underwater. They have unlatch 10 latches, slide a door open, and swim to a buoy. The second stunt is buggy as the ladies are chained up in a body bag full of crickets, red worms, super worms, stink beetles, and Madagascar hissing cockroaches and are shut inside a pitch black morgue drawer. They have to find which of three keys unlocked the padlock to their chains, unzip the bag, and push themselves out of the drawer. For the final stunt, the ladies are lifted off a dock and would have the crawl across a horizontal rope hanging underneath a helicopter. They must release 8 flags on the rope, drop into the water below, and swim to a buoy. The lady to complete this stunt the fastest would win the $50,000.
