Two years have passed and Gemma is bringing up her 15 year old son Tom alone so she is surprised to learn that Simon and Kate, now married, have moved back to the area so he can be nearer to Tom, whom he invites - along with some of Gemma's friends - to his wedding reception. Gemma decides to crash the affair with James, a handsome patient whom she has agreed to date and who turns out to be Tom's school-teacher, but, getting Simon alone, she drunkenly claims he still wants her, which he denies. Next day he visits her, telling her that she needs to move on and forget the past, and to prove it he persuades Tom to move in with him. He should have realized that this will, once more, set Doctor Foster on a further plan for revenge.

Stung by Tom's decision to live with Simon Gemma notes that he has been to see fellow doctor Sian and asks her for more information, being told that Tom has anxiety and sleeping problems. She then calls on Tom's school friend Max, who says that Tom hates her so she gets drunk, bumping into Simon, who photographs her and threatens to send the shot to Tom. She is cheered by sex with James but still believes Simon has sent somebody to spy on her. Whilst confronting him on how he afforded his new house she gets a call from Tom's school to say that he attacked Max for no reason.

Learning the reason for Tom's violence Gemma and Simon call a truce to support him. When Tom -after arguing with his father - returns to live with Gemma Simon comes after him and Gemma suggests a civilized dinner, where Simon reflects on old times, leading to a sexual encounter, though Simon thwarts Gemma's attempt to film it and send it to Kate. Gemma locates Isobel, the girl over whom Tom fought Max and finds that Tom drunkenly forced himself on her. Gemma is infuriated to discover that Simon knew and kept it from her and allows Tom to remain with her.

Before leaving town with Tom Gemma sends Kate a token to suggest Simon's infidelity, which, as planned, worries her, to the extent that she locates Gemma. During a confrontation witnessed by Kate's parents Kate appears to have the upper hand, despite Gemma telling her that Simon has put everything in his wife's name but she is still angry with Simon and lets him know it. The ever-vengeful Gemma builds on this, leading to Kate taking her daughter and leaving Simon homeless. Gemma would seem to be triumphant but Simon plans to tell Tom about his mother's scheming so Gemma aims to stop him.

Satisfied that she has ruined Simon's life Gemma returns home with Tom. Simon turns up but Tom refuses to hear him out though he is convinced that his father will never give up and Gemma panics when they disappear together after Simon threatens suicide. She tracks them down but Simon tries to turn Tom against her, firstly by accusing her of putting career before family and then playing the sympathy card by criticizing his parenting. Neither works but Gemma's recollection of being orphaned as a child influences her next course of action, not that it brings her a satisfactory conclusion to things.
