After 4 months pass, the hospital starts to reopen, and Steve announces that Caitlin has started robbing banks to raise money for R.O.A.R.: Revolutionary Order for Armed Rebellion. Steve and Jesse discuss plans to buy Barbecue Bob's, which is going out of business.

Steve and Jesse ask Mark for a loan to buy Barbecue Bob's. Caitlin and R.O.A.R. break Carter Sweeney out of prison, and the Sweenies kidnap Mark from his home. They ask Mark to figure out a way to obtain $100 million or they will keep blowing up stores and restaurants, killing innocent people. Mark agrees and comes up with an ingenious plan that works, involving a fake armored car.

Jesse begins to believe he may have been abducted by aliens and it takes Mark a long time to understand the how and why of his young friend's frightening paranoia.

However - she didn't act on the spur of the moment, nor had a motive for his death, she just wants to get inspiration from a real murder, and finds a clever way to have her (now deceased) Agent call her, providing herself with an alibi by having him phone her, and Dr Sloan and Amanda over hearing the call. She's delighted to see one of her dead-end suspects murdered in a copycat-like way, until it appears that she committed the second murder: she'll get the death penalty if neither murder is solved.

When Steve discards that, Harry falsifies a claim letter from the murderer and authenticates it on TV, causing a panic and high-profile resurrection of the task force, under Harry, who gets a call from the murderer, who already killed again. Now Harry shows Mark his theory the killer never stopped but worked in various oil-producing countries, painting clown-type faces according to local traditions, but Steve nails Sally Jenkins's killer, which compromises Harry's theory, yet captain Walter Newman won't even take him off the task force. Despite Harry resenting his only fiancée marrying Mark instead, they follow up on the travel theory. Oil company executive Howard Weber tells them which harassment-prone employee fits the travel pattern, so Steve and cancer-terminal Harry stake him and that is only the beginning...

After Steve shoots robber Nestor in the leg, the leader kills him and taunts Steve about the lousy LAPD equipment compared to his, and even dares Steve to kill him. The next day his next announced crime is robbing Steve's car to dump it in the river. Mark wonders if the robber's twitch is caused by the terminal disease Huntingdon's, and if it is somehow connected to his motive.

In this atypical episode there is no sleuthing, but Dr. Mark Sloane wrestles with medical ethics when, after turning away presumably terminal patients who asked him to help end their lives, he finds the new resident, Dr. Kate Delieb, systematically giving them what they want, by setting up a suicide-device in their home or even, administering the fatal injection. He confronts her, swearing he'll do everything in his power to stop her as he believes the Hippocratic oath's primary rule never to cause harm forbids killing absolutely. Nut after reflecting on his own action as a resident when his grand mother died in agony and his refusal as an established surgeon to do the same for Steve's terminal mother, he ends up pleading at a grand jury hearing against her that she may nevertheless be more honest and courageous then the morally correct inaction he observes himself.
