Mario is dryly bitter when his useless pa Vincent is wheeled in once more drunk, broke and grossly ingrate, flirting indecently and coming to worse. Angus gets encouraging news about post-comatose brother Mike, but is warned private concerns may never cloud his medical performance.

Ethan's battlefield experience is priceless again, but he gets stuck carrying up demented Blanche who mistakes him for her later war hero husband George, beyond help herself. Dixon remained stuck in the elevator to deliver a baby whose father agonizes outside the power-cut-locked door. Campbell is inspired to turn back recent sanctions.

Mario dreams of being assigned as club medic to the city's basketball team, but accuses 'silver spoon' rival Angus of nepotism as his father knows the team brass. Rowan Davies is brought in with a rare condition, which is alas found inoperable. Against Campbell's orders, Leanne uses the hospital helicopter to reunite a family one last time. Campbell urges Elliot to stand up to a bully patient. Ethan fights for a patient's life by using a therapy that can only save one in a million given genetic probabilities.

A slum mother brought in after a fire, initially with only one of her twin babies, refuses violently to bond with her healthy son after her daughter doesn't survive surgery. Teenager Jared is blamed for the fire by the janitor, along with his non-supportive workaholic absent father, but the fire chief concludes otherwise. Then a dreamer awakes from his ER nightmare.

Dr. Ethan Willis's exotic experience makes him rightly suspect a horrible truth: an atypical ER patient has spread a hemorrhagic fever, highly contagious and probably deadly. That calls for an immediate lock-down, protective 'astronaut' suits and intervention by the CDC, whose DD Gareth Reddick takes over from Campbell. The isolation must be enforced at all cost, to prevent infections elsewhere, but reconstruction suggests rightly that patient zero is not inside. Several infected staff members develop symptoms, including Mario, who takes charge 'inside', and Malaya Pineda, who quickly reaches the horrible terminal stage.
