FBI agent Clarice Starling has been held out of the field for more than a year since she previously captured and then killed serial killer Buffalo Bill whilst he was holding the FBI Chief Ruth Martin's own daughter hostage intended to be his next skinned alive victim until Clarice put an end to Buffalo Bill's murderous and torturous rampage. Chief Ruth Martin insists against the Psychiatrist's reservations to allow FBI agent Clarice Starling to go back out into the field investigation to solve the most recent murders of three (3) women who appear to be murdered by a new serial killer. Clarice who would prefer to stay in her preferred field as a Behavioral Analyst is pushed by Chief Ruth Martin to go right back into the middle of this new investigation based on her adept ability to understand how these serial killers think and act. Unfortunately for Clarice's superiors they soon realize that Clarice's grandmothers words of advice are more meaningful to her than the instructions of her superiors.

Clarice awakens in a ward bed, chemically paralyzed from the waist down and fitted to look like surrounding patients, groggy from sedatives and painkillers. Ward nurse Marilyn Felker threatens with a chemically induced coma and a ventilator if she misbehaves, then sedates Clarice with more painkillers. Alrted by Ardelia over Clarice's disappearance, Clarice's teammates try and find out what happened. Felker's continuous injections of sedatives and painkillers cause various vivid hallucinations (especially featuring her fatally shot cop-father), to get her to divulge - on Felker's boss's orders- what the FBI knows and where Rebecca Clarke Sherman is. Starling hits back by psychologically targeting Marilyn's childhood trauma but pushes too hard, so Marilyn paralyses Starling's lungs, intubates her and puts her on a ventilator. Vicap comes to the clinic looking for her, Marilyn reinforces Starling's patient disguise, they fail to spot her and leave. Clarice and Marilyn spar verbally during Starling's bouts of lucidity. The FBI team finally makes a connection that enables them to thoroughly search the clinic and assemble a strike team. Starling overcomes the drugs in her system and escapes by forcing Marilyn to save her comatose sister from a drug-induced heart attack. The FBI arrive and rescue Starling and the sister but fail to stop Marilyn from committing suicide via a fatal drug overdose. Starling is reunited with Ardelia but deeply traumatized..

Clarice tries hypnosis to remember who she saw with Marilyn Felker during her attempt to escape from captivity, but her memories intermixed with the confrontation with Buffalo Bill. Krendler enforces leave to recover from the ordeal. Clarice, determined to identify her attacker, asks Attorney General Martin to overrule him, which she agrees to if Starling join her and Catherine for dinner. Ardelia sent DNA samples from Clarice's fingernail for analysis, but the results are inconclusive, but retrieves the sample and finds the result was faked. Krendler meets a lawyer to represent him in his divorce case, determined to seek full custody after his son pages him and he finds his wife passed out from drinking. Over dinner, which Catherine initially refused to attend, she tells having tracked down Buffalo Bill's mother and after Clarice shot Bill, Clarice cried long before calling for help. Clarice cannot remember, goes under hypnosis again and finds that Catherine was right-with her memory cleared, she recalls the face of the man with Felker: Krendler's new divorce lawyer.
