Episode 1

A lot has changed in the Kleist household. Lisa and Michael have finally separated, and Lisa has moved to Paris with her daughter Rosalie. The upstairs apartment in the villa now houses Christian's son Piwi, his girlfriend Anke, and one-year-old Ben. The practice is now financially secure again: Dr. Timotheus von Hatzfeld was rehabilitated with Christian's help and received his license as a pediatric radiologist. All debts have been paid off. Two cases are particularly troubling Christian: a male model suffering from strange paralysis symptoms, and especially Nina, Paul's school friend, whose tonsillitis turns out to be a severe case of diphtheria. A race against time ensues, as Nina needs an antiserum within 24 hours. But no matter what the doctors try, the serum cannot be found in Europe. The next day, Christian travels to Weimar to attend a medical conference. There, he meets a woman who captivates him. They spend the rest of the evening and the night together.

Michael has recommended a doctor he knows from his student days to Christian as Lisa's successor. Christian invites her to an interview and experiences a pleasant surprise: It's the woman with whom he spent a passionate night during the conference in Weimar. At the same time, Christian has to convince two patients to seek treatment: Piwi's former fitness trainer, Robert, is taking a thrombosis lightly and doesn't want to undergo surgery. And the case of Mrs. Lorenz is also causing Christian a headache. Mrs. Lorenz is a spry widow who is looking after her grandson when her daughter is admitted to the hospital with blood poisoning. The emotional strain is too much for Mrs. Lorenz: Wanting to be strong for her grandson and her daughter, she arrives late to see Christian with heart problems and suffers a heart attack. Meanwhile, at the villa, Clara receives a message from her biological father, Justus. He sends her a letter in which he promises to support her financially. The money he wants to transfer to her, however, is intended for her education. Christian thinks this is very decent of Justus, but he's worried about Clara, who is spending the money on everything and neglecting her studies. He would prefer that she confront her biological father - but Clara wants nothing to do with Justus.

Tanja receives confirmation from the licensing office. She can now officially succeed Christian's daughter, Dr. Lisa Kleist, and take over the gynecology department in the group practice. Christian and Tanja have prescribed "professional distance" in their practice. Easier said than done, because when she misses her last train to Weimar at a small welcome party, Piwi naturally offers her the chance to stay overnight in the villa. Masal and Bahar Asidi came to Eisenach as refugees from Afghanistan and live in appropriate accommodation. Although Masal has severe upper abdominal pain, as a devout Muslim, she does not want to be treated by a man. Tanja diagnoses gallstones, but these cannot have caused the subsequent anaphylactic shock. Christian is also puzzled by Svenja Kesting, a friend of Anke's: What could have caused a stroke in a healthy young woman? Christian and Dr. Michael Sandmann attend a climbing taster course. Michael is clearly enjoying the climb, but when he's momentarily careless while abseiling, he loses his footing and crashes into the rock. After a brief loss of consciousness, Christian diagnoses a mild concussion. He would like to examine Michael more closely, but he refuses.

Her next patient immediately presents her with a challenge: The young woman is experiencing dozens of unwanted orgasms, which are increasingly leading to social isolation. And Christian also has his hands full: A young boy has been suffering from recurring gastrointestinal problems for several weeks. Could the cookies from the bakery where his mother works be to blame? In addition to the patients in the practice, Christian is worried about his friend Dr. Michael Sandmann. He suddenly develops paralysis in his right hand during an operation. A long-term consequence of the climbing accident? Christian and Timo find evidence of a hemorrhage in the spinal cord, which can lead to progressive paralysis and cannot be cured. Is Michael's career over? Meanwhile, Clara contacts her biological father in Canada. When Justus invites her to visit him at his horse ranch, Clara is torn. Her adoptive father Christian is once again at her side with advice and support.

Her husband, Oliver, and their children, Merle and Nico, are very worried. When Kerstin comes home that evening with a cast on her arm, she is very upset: she can't remember anything again. Oliver turns to Christian, who had fixed her broken arm the day before, and asks him for help. After an MRI with Dr. von Hatzfeld, it is clear: Michael is suffering from a herniated disc in his cervical spine, which requires urgent surgery. Michael is hesitant, fearing side effects from the surgery. If something goes wrong, he will never be able to work as a doctor again. Just as Christian has convinced his friend of the necessity of the surgery, Piwi remembers a patient with similar symptoms and thinks of another illness. Paul, without a driver's license, has an accident on a friend's scooter. He asks Tanja, who witnessed the accident, not to tell his father. But she doesn't want to lie to Christian and expects Paul to tell his father. Tanja also faces another problem: She has to move out of her apartment in three weeks. Christian offers her the chance to move into the villa, but Tanja thinks that's too fast.

17-year-old Caroline Beermann relies on regular dialysis and is waiting for a kidney transplant. Only Caroline's sister, Julia, is considered a suitable donor. But the family is undecided: Should Julia, who still has her whole life ahead of her, risk her own well-being? While Caroline's parents are still discussing the matter, Caroline decides against her sister's kidney. But suddenly, her condition worsens dramatically, and the parents are faced with the most difficult decision of their lives. In order to spy on her mother's new lover, Tanja's daughter Lotte makes an appointment with Christian under a false name and with fabricated symptoms. But Christian quickly sees through her - until Lotte suddenly actually feels pain. Michael's hand problems have healed, but he's afraid it might start shaking again. Therefore, he delegates operations to his assistant. What brings Professor Bender into action: Michael is the clinic's figurehead, he has to operate - the clinic group wants to go public and the numbers have to be right.

At the same time, Timo receives an "immoral" offer from Professor Bender to move to the hospital as head of radiology. Timo hesitates. Will he let Christian and the practice down?

At Villa Kleist, everyone is happy to welcome their temporary roommate, Tanja. Christian, in particular, realizes he doesn't want to be without her anymore. His feelings for Tanja are growing stronger and stronger, and he can imagine her moving in with him forever. Meanwhile, Christian's patient, Roman Lubitz, has injured his hand while gardening. Mr. Lubitz doesn't attach much importance to the scratch - until Christian identifies the injury as a dangerous animal bite. And the second case also keeps Christian on his toes: Gertrud Meerwein is forgetting appointments with her granddaughter, and her general health isn't the best. Initial tests indicate dementia, but the elderly lady doesn't want any further examinations. Can Christian change the frightened woman's mind? At the hospital, Michael is surprised that Professor Bender unanimously approves his recommendation for a junior doctor. The chief physician quickly realizes there's a catch: The applicant is Bender's daughter, Dorothee.

Franziska, a tourist in Eisenach, asks Christian for an antibiotic that her gynecologist always prescribes when a bladder infection is coming on. After initial hesitation, Christian gives her the medication, but Franziska's health deteriorates rapidly over the next few hours. An MRI scan reveals that Christian is able to rule out a stroke, but what caused the sudden speech slur and paralysis? The MRI scanner at the group practice needs repairs. The bill presented by managing director Georg Wehner is quite substantial: almost twice the estimated amount. Christian tries to talk to Wehner, but he is stubborn. Either the bill is paid or the MRI scanner is dismantled. Suddenly, Wehner himself is dependent on the MRI and realizes that people's health is more important than insisting on financial demands. Tanja has now found an apartment in Eisenach, although it still needs to be renovated. Lotte is staying with a friend in Weimar in the meantime so she can finish the school year. But this friend's mother takes a job in Bavaria and leaves Weimar immediately. Without Tanja's knowledge, Lotte goes to Berlin to stay with her sister and father, but Holger's apartment is too small. So Lotte comes to Eisenach and moves into Clara's room-who, everyone believes, is still in Canada. But Clara, with an apprenticeship lined up, returns earlier than planned and finds her bed occupied.

During a fishing trip, a branch fell on David Meissner's head. He was briefly unconscious, but now everything seems to be back to normal. Suddenly, however, David's condition changes dramatically: Due to toxins in his body, he is at risk of life-threatening kidney failure. Tanja's patient, Maren Viernau, is also fighting for her life: After arriving at the practice with menstrual cramps, Maren unexpectedly suffers cardiac arrest. But what triggered it? Christian is happy because Tanja seems to like it at Villa Kleist. Their daughter Lotte, who has been moved into an attic room after Clara's surprise return, feels quite different. Although everyone is trying hard to make it easier for Lotte to settle in, tensions arise between her and Clara. Tanja fears that she has overwhelmed family life with Lotte's arrival. Bernd and Inge manage to calm the waters between Lotte and Clara somewhat for the time being: Lotte appears more conciliatory and is even pleased with the family's successful surprise of making the attic room cozy for her. But then there's another heated argument with Clara, which Christian can only calm down with considerable psychological skill. He and Tanja realize that the new patchwork situation will continue to pose major challenges for both of them in the future.

Christian is faced with a difficult patient: Timo's father, diplomat Alexander von Hatzfeld, fears he has Alzheimer's disease. Treatment is proving difficult, as the rift between father and son is deeper than ever. Alexander von Hatzfeld believes his son should use his talent for greater things than being a radiologist in the countryside. Timo, on the other hand, accuses his father of being excessively strict and lacking in understanding of his wishes. Despite all their differences, Timo knows his father well enough to quickly notice that something is wrong. Since his father doesn't want to admit anything, he and Christian come up with a plan to get him to talk. At the same time, Tanja is called to see Hannah, a heavily pregnant patient whose child is in a breech position. When Tanja arrives and performs a final examination, she discovers that the child has turned and nothing stands in the way of a home birth. But then unexpected complications arise. After Clara's stay at her biological father's horse ranch in Canada, she now knows what she wants to be: a horse farmer. However, to begin her training, she would have to leave Eisenach. When Clara has doubts, Christian supports her plans and promises her that, no matter where she is, Villa Kleist will always be her home. Meanwhile, the situation between Paul and Lotte continues to escalate: Lotte bullies Paul at school, which leads to him skipping school for the first time in his life - leaving Tanja and Christian in the midst of patchwork chaos.
