The case of the young soccer goalkeeper Sylvia Hoffmann is of great concern to Christian. She took a blow to the sternum during a brilliant save in the decisive match against relegation and has been suffering from dizziness and extreme pain ever since. Christian then arranges blood tests and a CT scan - but the case remains a mystery. Only an MRI could shed light on the matter, but the machine in the hospital is booked up for days and the last remaining radiologist in Eisenach has given up his practice due to age. In the meantime, Sylvia's condition deteriorates rapidly. Christian immediately calls the emergency services, but it is too late: despite all measures, the young woman dies during transport. Christian is devastated. A young woman - talented, athletic, with her whole life ahead of her - is dead. Perhaps he could have saved her if he had had his own MRI machine. Christian realizes that he is at a crossroads. He cannot simply carry on with business as usual. Due to the strict austerity measures of the hospital administration, the days are over when he could rely on the clinic's resources at any time with the help of his future son-in-law Michael Sandmann. There are only two options: quit or adapt to the circumstances and reposition himself. Specifically: modernize and grow. The idea of a joint practice is born. Together with Lisa, now a qualified gynecologist, and Michael and Piwi as physiotherapists, Christian wants to start again from scratch. But will the children embark on this adventure? In his desperate attempt to impress at school, he uploads a porn film to the boys' chat group. Christian's youngest son didn't expect that Kai would immediately forward the dirty film to the girls. Lisa and Michael's daughter is also causing concern. She's ill, and since Michael, as the newly appointed chief physician at the clinic, has his hands full, Lisa has no choice but to take Rosalie with her to the practice. But the initial sore throat develops into a life-threatening abscess in her throat. A possible operation carries high risks - but can they wait any longer? Piwi proposes romantically to Anke. But Anke declines. Piwi is shocked - he really hadn't expected that. When Anke then reveals to him that she's been offered a trainee position at a luxury hotel in Singapore and intends to take advantage of this unique opportunity, Piwi decides to leave their practice and accompany his girlfriend to Asia. On the other hand, Clara's luck is back on. She isn't pregnant and is very relieved about that. After a long conversation, in which Christian was much more understanding than expected, everything is back on track between her and Christian. When a Berlin music producer unexpectedly offers them a record deal, Clara and her band "Pink Slip" are on cloud nine.

The atmosphere between Christian and Piwi is becoming increasingly icy, as Piwi is determined to give up his responsibilities at the practice so he can accompany Anke to Singapore. He doesn't want to live solely for work like his father: Anke and his private life should take priority. While Christian's youngest son, Paul, is having trouble at school, his partner, Anna, is struggling to secure new clients after her main client goes bankrupt. Fortunately, she's able to mitigate a costly warning for Paul's illegal downloads, but the Kleists' financial situation remains difficult. After Piwi's accusations and Paul's problems at school, Christian increasingly doubts his abilities as a father. But work stress and money worries soon take their toll on him so much that he secretly resorts to tranquilizers. Meanwhile, Clara has a clever idea to actively help her family out of their financial predicament.

Anna's financial situation is also becoming increasingly difficult, so she seeks legal representation at the district court with the help of a former mentor. Her first assault case presents her with a moral dilemma. Nora, on the other hand, has every reason to be happy, as Lisa hires another medical assistant for the practice to provide relief. Meanwhile, the erotic tension between Lisa and Timo grows ever stronger and ultimately erupts in an affair. Lisa regrets this shortly afterward, but she can't bring herself to tell Michael the truth. At the same time, Christian and his colleagues Lisa and Timo are investigating a mysterious case: The young gymnast Melanie is about to compete in an important competition and is complaining of chronic exhaustion. But does it make sense to take legal action against his boss, Professor Bender? A discussion ensues between Piwi and Anke, during which he apologizes for his immature behavior. Anke is willing to forgive Piwi, provided he finally takes responsibility for their relationship and for the Kleist practice. She wants to start her trainee position in Singapore alone. In the joint vote for Piwi's future in the practice, Lisa and Timo vote in favor, but Christian is still unconvinced. The mood in Clara's band, "Pink Slip," is tense: Clara and Sabrina are still struggling with the fact that they turned down the Berlin music producer's offer so that their friend Lauri could remain in the band. Lauri herself realizes that she's not the best guitarist. An unexpected family reunion: Inge and Bernd suddenly appear at the villa's door. They have unexpectedly cut short their world tour. After Christian informs the health department, it soon becomes apparent that other people in Eisenach are also exhibiting the same symptoms. Is an exotic virus spreading in the city? Finally, Bernd rushes to the practice with the critically ill Inge. Christian diagnoses her with recurrent pneumonia, which requires immediate treatment. But before Inge can be admitted to the hospital, the department imposes a quarantine on the practice - no one is allowed in or out. This also has consequences for Piwi, as he actually wants to take Anke to the airport to see her off before her trip to Singapore. At the practice, the medical team is fighting on several fronts: Dennis' condition is steadily deteriorating, Carmela is about to give birth, and Anton is losing his nerve, while Inge's condition is becoming increasingly dire. In a race against time, Christian, with Michael, Lisa, Piwi, and Timo, tries to quickly uncover the cause of the mysterious illness and end the quarantine - because Inge isn't the only one in acute danger.

Carola Kämper, a taxi driver in Eisenach, visits Lisa's practice because of pain, fatigue, and dizziness. The symptoms, which Lisa initially attributes to menopause, are more serious than expected. Shortly after her visit to the practice, Carola suffers a fainting spell while driving her car, almost leading to a serious accident. The single mother now fears losing her job and no longer being able to care for her two sons. When Carola finally loses her sight after another fainting spell, she fears she will go blind forever. The atmosphere between Michael and Lisa is tense: Since his reprimand from Professor Bender, Michael has been under enormous pressure, which he unfortunately brings home with him all too often. Lisa, on the other hand, is tormented by her guilty conscience. She finally confesses her affair with Timo to Michael. For Michael, everything is now perfectly clear; he saw the signs, but couldn't interpret them. Hurt, he turns away from Lisa. Things aren't going well for Christian - the fact that Anna is away in Munich on business and can only be reached by voicemail is bothering him. He also continues to take pills to combat his work stress and insomnia. Piwi doesn't miss the fact that his father is now developing an addiction problem and confronts him. The family council meets in the kitchen, and a discussion ensues.

At the clinic, Professor Bender continues to put him to the test by forcing drastic budget cuts on him. Michael vents his anger and gives Lisa a tough ultimatum: either she leaves the practice or Timo must leave. Lisa, however, refuses to give in to this demand. Christian is also unwilling to fire Timo. Michael talks to Christian and says that it was a mistake to have always put Lisa's needs before his own in recent years. Nevertheless, he doesn't want to lose her. When Christian surprises Anna with a romantic hot-air balloon ride, she's only concerned with her business appointments and cancels. A heated argument ensues. There was a lot of chemistry between Clara and her friend Sabrina after the school trip. But even Lauri has no idea that the two are growing closer. Clara is confused - has she fallen in love with Sabrina?

However, the stress of the impending legal dispute is so severe for Maren that she is experiencing bleeding and severe circulatory problems. Lisa and Christian make it clear to Maren that her own tension could jeopardize the birth of her child. After Maren's husband, Tobias Werner, accidentally breaks Werner's nose during an argument, he threatens to use the injury as leverage in court. But the situation is slowly becoming unbearable for Stefanie, too, and so she asks Anna to intervene as a mediator in the case. Meanwhile, the disagreement between Christian and Anna over the legal dispute culminates in a heated argument. When Christian hastily accuses Anna of career-oriented motives, she turns away from him, hurt. Michael has temporarily moved into a hotel after Lisa's affair with Timotheus von Hatzfeld. Lisa is perplexed because she still hasn't gotten through to him. Their daughter, in particular, is noticeably suffering from Michael's absence. Christian soon has to present the Kleusbergs with a devastating diagnosis. After Michael makes it clear to Lisa that he wants a divorce, she is devastated. On Christian's advice, Michael forces himself to have another conversation with her. Old wounds resurface, but both realize that they still love each other. Clara, confused by her feelings for Sabrina, seeks advice from her older brother: How can she figure out whether she's a lesbian or not? Piwi advises her to try again with a boy to finally come to terms with her sexuality. When Clara meets up with her ex-boyfriend Yannick for this purpose, they are secretly observed by Sabrina, who is completely shocked. Things are flaring between Christian and Anna. Christian is convinced that Anna has only been concerned with herself and her work since her time in Munich. But she's particularly unwilling to accept this accusation from Christian. Clara alerts Christian, who manages to save Yannick at the last moment but is also furious about his irresponsible behavior - after all, Yannick is of legal age, while Clara is not. Christian urgently warns Yannick of the dangers of his drug experiments, but he shows no remorse. During one of his emergency medical calls, Christian must once again treat the unconscious Yannick. Together with Timotheus von Hatzfeld, he investigates the causes of Yannick's breakdowns and just barely prevents them from getting worse. After breaking up with Anna, Christian finds it extremely difficult to adjust to single life again. He tries to fill the emptiness in his everyday life with more work. He finds suitable company with von Hatzfeld, of all people, because he has to accept that Lisa is blossoming in her marriage to Michael. Piwi, on the other hand, is very worried about Anke, because she seemed strangely depressed during his last video call. She hasn't contacted him since, and Piwi becomes suspicious - is Anke having an affair and doesn't know how to confess it? He can't stand the uncertainty any longer and calls Anke's hotel in Singapore, but she's no longer staying there. He decides to fly to Singapore to find out for sure. On the way to the airport, Piwi receives a surprising cell phone message from Anke: She's on her way from Singapore to Eisenach. When the two arrive back at the Kleists' house together, a big surprise awaits the family: Anke is pregnant.

Due to the onset of premature labor, Anke is in danger of losing her child in the 30th week. Lisa is able to call an ambulance just in time. From now on, Anke must remain in bed for the rest of her pregnancy. To spare her the long hospital stay, Christian arranges for Anke and Piwi to move into the villa. The constant lying in bed and doing nothing is particularly difficult for her. During her visit to Eisenach, Susanne Dieken learns that her younger sister Marie is pregnant. While Marie and her husband are happy that their long-held wish for a child is finally coming true, Susanne is not at all enthusiastic. After their mother's untimely death, their family doctor had warned the two sisters that the life-threatening hereditary disease lupus erythematosus is passed on to every second generation. This information later turns out to be false: Based on Susanne's symptoms, Christian suspects that her lupus erythematosus has long since broken out. With Michael's support, he orders an ANA test. The result is devastating. Although Kleist promises his patient that he will slow, if not stop, the progression of the incurable disease with appropriate medication, Susanne no longer sees any prospects for herself and fears she will die a death as painful as her mother's. The close relationship between the two sisters is put to the test again when Marie's genetic test reveals that she and her child are not affected by the genetic predisposition. Against Christian's doctor's orders, Susanne begins her return journey. Already on the way to the train station, she suffers another attack and collapses. On Christian's advice, Michael arranges for cortisone therapy, which saves Susanne's life at the last moment. A mix-up over a harmless rose causes Michael's jealousy to flare up again. Michael is desperate for von Hatzfeld to finally disappear from Lisa's circle and hopes to be able to lure him away to a clinic outside of the area. But the more Michael researches him, the more he realizes what an outstanding doctor Timotheus von Hatzfeld is. During his investigation, Michael also uncovers a secret that could not only end the young radiologist's career in one fell swoop, but also endanger the practice.

Things are in chaos at Kleist's practice: Professional wrestler Lukas Bukowski is being treated by physiotherapist Piwi for bruises and contusions near his spine. Initially, Piwi simply prescribes a break from training. When Lukas Bukowski again complains of pain in his kidney area, he consults Christian for an internal examination. Despite symptoms of inflammation, the cause remains a mystery: The results confirm neither an abscess nor kidney stones. Shortly before the wrestler collapses in the practice, radiologist Timotheus von Hatzfeld discovers in the CT scan that Lukas has blood poisoning, which could not be diagnosed due to mixed-up lab results. Christian must now perform emergency surgery with Timo's help. Meanwhile, Christian has learned that Timotheus has been practicing as a pediatric radiologist without a license. When Christian sharply reprimands him, Piwi remorsefully admits that his lie has jeopardized the practice's existence. He promises to make amends for his misconduct. Christian is struggling with the prospect of being dismissed without notice because Timo's skills as a radiologist are excellent. Although it was Michael who uncovered his lie, Timotheus now asks him for help and shows him a meticulously kept logbook documenting his qualifications. Von Hatzfeld must somehow prove that his former mentor, Professor Schleimann, stole his research ideas and then wanted to get rid of him. The Medical Association has therefore wrongfully denied him certification as a pediatric radiologist. Michael is trying to find witnesses from Timo's former clinic to prove Professor Schleimann's fraud. Meanwhile, Anke is causing concern for the family: she is going into acute labor. Michael has to deliver the baby in a dramatic emergency operation. The prematurely born boy survives, tiny and weakened, and has to be cared for in an incubator. The Kleists are both relieved and worried: Anke is doing well, but will the premature baby survive? The arrival of the new family member is appropriately celebrated with a large garden party. Christian welcomes the new Kleist in his loving way. He is happy that his family is doing well.
