Just as Vito is about to succeed in his transition to civilian life, an old crime comes back to haunt him that will change the course of his legacy.

With Vito now in a maximum-security prison, Declan must step up to run operations and is stunned by a fight for the throne from Vito's son, NICO JR. Declan promises Vito that his first priority will always be to protect the Rizzuto family, but Nico Jr.'s actions make life and business deadly for Declan and the rest of the organization.

Vito's father, NICO SR, is forced to take back the reigns as acting boss and orders his grandson to return to civilian life. Just when things seem to be going smoothly, a turf war breaks out and Declan finally takes on the role of the organization's strong leader. Tragedy strikes the family and threatens to rip apart everything Declan has worked for.

After tragedy strikes his family, Vito is released from prison. He goes into a self-imposed exile and cuts Declan out. The syndicate's alliance is threatened by Vito's refusal to engage with his associates. When Declan is finally invited back into the fold, he convinces Vito to "clear the board' of anyone who may have crossed them.

Vito and Declan go on a rampage of revenge, murdering anyone who may have had a hand in the deaths of Nico Jr. and Nico Sr. Vito abandons all previous ideals of peace and deal-making. Declan runs the entire operation but there is still a mole in the organization that they must smoke out.

Vito and Declan reclaim their titles as the undisputed mob bosses after killing all their enemies. Declan is ready to move things to the next level - to go legit just as Vito had planned - but prison and tragedy have altered Vito's mindset. What will happen to the organization now that its two leaders see their futures so differently?
