Matt (Cuba Gooding Jr) and Shelby (Sarah Paulson), a interracial couple decide to move to North Carolina and get away from the big city for peace and quite, to start a new life until Shelby starts seeing strange things that haunt her day and night.

Shelby witnesses a scalped man with pig head being roasted in the woods. When she is spotted by a woman, she flees while the group hunts her down, and she faints in front of Lee's car. At the hospital, Shelby is examined to see whether she is on drugs, but she believes everything was staged by the locals to force them to leave the house. Mason brings Flora to visit Lee, and soon the girl befriends an imaginary friend, Priscila. Soon Shelby and Matt find the place where the stranger was roasted, and they call the police. Late at night, Matt wakes up with the telephone ring and he finds the cord disconnected. He sees two nurses shooting and killing an old woman. He calls the police and soon he believes he may have brain damage caused by the aggression in Los Angeles. Mason arrives to take Flora back home and she is hidden. When Lee and Mason find their daughter, she says that she was exchanging her favorite doll with Priscila to avoid her family is killed. Now she says that she will be the last to be killed, and Mason leaves the house with Flora and tells Lee that she will never return to that house. Mason and Matt investigate the cellar and find a VHS camera with a tape recorded by Dr. Elias Cunningham that describes the evil forces that are trapped in the house. Matt tries to undo the business with the bank, but fails; therefore, the couple is forced to stay in the house. The drunken Lee abducts Flora, violating the custody deal, and brings her to the house but Shelby talks to Mason and resolves the problem. However, Flora disappears in the woods.

The search for Flora begins and lee begins to face more troubles than before. Clues lead Shelby, Matt, and lee to a farm where they discover a horrid site. A medium attempts to help Lee find Flora, but instead contacts a darker spirit from the 1500's.

Shelby and Matt are at home discussing what happened at the woods, when suddenly they are attacked by a man with a pig head. Out of the blue, Dr. Elias Cunnigham appears and protects them, saying the word "Croatoan". He explains that the word is dark power and shows Matt and Shelby the historic of the house that he keeps to write a book in the future. He shows that every blood moon in October is the blood moon and during six days, the ghosts become more powerful, and the cycle starts that night. Matt and Shelby mention the name of Pricila and Elias tells them that he knows where she likes to be. Soon they are attacked, Elies is murdered, and they return home, where they stumble with Crocket. He tells the story of The Butcher and her colony, and how she poisoned and killed the settlers. Now Crocket intends to help the Millers to retrieve Flora. Will they succeed?

A historian discloses the story of the builder of the Roanoke House, the cruel gay Edward Philippe Mott, who builds the huge house to expose his artwork for himself. His lover is the black slave Guinness, who lives with him. When The Butcher creates havoc with his paintings and destroys one of his masterpieces, he blames his servant slaves and locks them naked in the root cellar until the guilt confesses the destruction. But soon he is murdered by the Butcher's mob, Guiness escapes but he does not tell anyone that his servants were locked in the root cellar. Nowadays, Matt calls the police since the mob is outside the house threatening Shelby, Flora and himself. Suddenly, Edward appears and shows them a means of escaping through tunnels. But they are captured by the Polk family. They see Elias wounded but alive with a severed leg below the knee. Mama Polk says that she made a deal with The Butcher and will deliver them to her. While driving them to The Butcher, Matt reacts but they are recaptured by the Polks. Mama Polk uses a sickle to cut Shelby's foot to prevent them from running. Meanwhile, Lee is released and asks a police officer a ride home. The Millers and Flora are delivered to The Burcher and her mob and now only a miracle will save them.

After the success of the show "My Ragnoke Nightmare", the producer Sidney James convinces the network president to reunite the Millers, Lee and the cast in a reality show in the infamous house during the blood moon and call it "Return to Ragnoke: Three Days in Hell". Sidney has bought the Ragnoke House from Matt and Shelby and is installing hidden cameras for the show, as well as special effects to scare the participants. He also wants to stress Lee since the murder of Mason is unsolved. His assistant tells him that Shelby does not want to participate, since she has split from Matt. The actress who played The Butcher is insane and will not be part of the show. When Sidney and Diane arrive at the location, they learn that an accident had happened with one crew member. When the participants arrive, bizarre things happen and one of them is murdered by the nurse sisters.

The producers catch a glimpse of Rory's demise and go outside only to suffer the same fate. Now all the actors are alone in the house, with only the cameras as witnesses. Agnes returns still in The Butcher character and attacks one of the returning house guests. The survivors split in two groups, 3 stay in the house and 3 go for help through the tunnnels, but no matter where they go, noone is safe.

Shelby and Dominic decide to leave the house throughout the tunnels, but they do not succeed and retreat to the bathroom. Suddenly, Shelby slices her throat regretting for killing Matt. Meanwhile, Lee, Audrey and Monet are captured by the Polks and Mama Polk seasons one of Lee's thighs. Jether slices her thigh and Mama forces Shelby and Monet to eat the flesh. Jether is horny and Lee seduces and strangles him and succeeds in escaping. Monet also gets rid of one of the Polks and flees. Mama Polk pulls one tooth of Audrey with a pliers, but Lee arrives, and releases Audrey that kills Mama Polk with a hammer. They run to the house and they see Matt and Shelby's bodies. They blame Domenic for the murders and Lee locks him outside the bedroom, and Dominic is murdered by the Piggy Man. On the next morning, they decide to return to the farm to retrieve the videos, and when they are leaving the house, they stumble with Dylan.

Audrey and Lee hatch a plan to rescue Monet from the Polk compound. A new actor shows up to the house per Sidney's request; three young adults enter the woods to shoot footage of the blood moon and the history of the house for the web. Meanwhile Lee, Monet and Audrey remain on the run.

After the death of all the cast of Ragnoke, Lee is prosecuted but the jury considers that she is not guilty. Then she goes to court again for the murder of Mason, but she is considered not guilty again. However, Flora does not speak to her anymore and Lee goes to the show with Lana Winters to be interviewed by the host. Famed Lana Winters interviews Lee Harris. When she learns that Flora is missing, she returns to the Ragnoke house where she meets three "spirit hunters" and Ashley Gilbert during the blood moon. She advises them to leave the place, but they do not believe her warning. When Lee finds Flora, she must take a decision.
