NYFD station chief Owen Strand, the lone survivor of his Manhattan firehouse on 9/11, initially declines a request to move to Texas to rebuild the fire-wrecked crew of Austin city's firehouse 126. After a diagnosis with probably terminal but mid-term-manageable cancer and his doting, ex-junk twen son and fellow fireman T.K.'s failed suicide attempt by drug relapse after a year over his gay fiance's confession of infidelity, California-born Owen decides they both need a fresh start and accepts to move, for good. He recruits a crew form all over the US and makes lots of changes, while winning sympathy. T.K.'s reluctance to leave his friends and sophisticated Manhattan for redneck Dixieland soon wears off, as they find unsuspected modernity all over Austin, and T.K. a hot Latino crew-mate as gay playmate.

TK hastily suspends his gay affair with hit cop Carlos, whose home dinner suggests he wants a relationship, unlike TK. EMT captain Michelle answers a personal call for help aiding a young boy with an asthma attack; the grateful family has a clue about her sister Iris's disappearance three years prior. She later consults a 'curandera' (healer witch) for advice, while her kin won't touch more false hope. Judd and Owen bond over their therapies for post-traumatic stress disorder (PTSD) viz. lung cancer.

While the team rushes to save a farmhand suffocating in corn within a grain silo, TK disobeys orders to save Muslim colleague Marjan; videos of the rescue showing her hijab had fallen off go viral, an embarrassment at her mosque. TK gets into a dispute with Judd who calls his daddy's baby and instigates a fight at a bar, but in the aftermath makes up with Carlos, confiding his drug past into the kind cop. The EMS team diagnoses a collapsed sirloin eating contestant with hyponatremia, "water intoxication", causing a distended bladder. The team hospitalizes a woman who fell from her seventh-floor balcony while 'demonstrating' a new yoga pose. Against her restraining order, Michelle confronts her sister Iris' old boyfriend Dustin, while Owen gets chemotherapy and advice for dealing with his lung cancer, not alone; the two later share tequila at the bar. Drunken, Owen assaults Dustin, who nevertheless the next day arrives to help Michelle, albeit with unpleasant truth.

The team answers to a brawl at a male and queer strip club, where Paul helps Josie, who got hit in the eye with stripper glitter, next lets TK push him to date, but when he reveals his gender identity she cannot deal with it. TK and Carlos take him out dancing to get over her. In chemotherapy, Wayne warns Owen that impotence is a likely effect. Owen meets and seduces psychology professor Zoe, but cannot get erect, so she advises erectile dysfunction is usually best addressed by relaxing, that proves to be better then the 'wonder injection' he pressed his doctor to prescribe. Carlos's police patrol responds to a women's shelter where a misogynist incel protester hit in the head reveals to the paramedics he has calcium pyrophosphate dihydrate crystal deposition disease( CPPD), perceived like extreme arthritis. En route to the hospital he needs defibrillation, which chemically reacts with his ingested medication into toxic vapor causing the ambulance crew to pass out, and the vehicle to flip. At a bovine breeding facility, a disgruntled customer sets fire to distract from his theft of overpriced semen from the bull he raised.

T.K. helps Owen train for a record time at the Candidate Physical Ability Test (CPAT), which he bet his command on, but keeps fainting from chemo. After Grace prods Judd to assist Owen, who helped him, in dealing with Billy and the CPAT, Judd confronts Owen for letting his ego make the CPAT wager. Playing golf in a storm with Owen, Billy refuses to back down but is struck down by lightning. Owen carries him to safety, which the Deputy Chief equals with the CPAT challenge. Handyman Chad's epileptic seizure is mistaken for electrocution by his client, who beats him with a piece of wood till he drops a cable, the apparent cause. Cop Carlos helps Michelle follow up on kid sister Iris' disappearance through his superior, APD detective Washington.

In hospital Owen sees TK awaken from coma after accidental gun shot wounds; recovering, he seeks to find meaning in his life. A man spelunking with his son 'to learn courage' gets trapped upside-down; the team eases out and lifts him from the 80-foot cave. TK makes sure the boy who accidentally shot him isn't traumatized. Judd's father Stuart Ryder stubbornly denies the trouble living on his own so he and Grace try to help; he only listens after severely cutting himself falling on a glass-topped table. A young boy gets a toy car stuck up his nose but refuses to go to the hospital, Paul intervenes to get the toy out; later the team finds his curious father got the same toy stuck up his nose.

Austin, We Have a Problem

Chaos hits Austin when a solar storm causes the electricity and power equipment to malfunction. The 126 team rescues the passengers of a light aircraft caught in the lines of high voltage electric towers while transporting a sick man for a liver transplant. During an outing with Carlos, TK questions his relationship with him after Carlos begins asking. Malfunctioning traffic lights causing accidents, they rush to help people before the 126 arrives. In the homeless camp, Michelle discovers her lost sister Iris living there, schizophrenic. Michelle and bossy mother Theresa Blake try to get her back home but she prefers the camp. With the lines scrambled, Grace gets a call from the damaged space station, and at request connects its last astronaut, dying from radiation poisoning, with his family to say goodbye. TK confesses to the rest of the team his addiction forced Owen to drag him along, and shares he finally chooses for himself to be a firefighter after all. Later, he makes up with Carlos.
