What would a season finale be without complications? For Royal Pains' Divya (Reshma Shetty), that could mean early labor pains. The physician's assistant, pregnant with her ex's child, suddenly finds "there may be something wrong with the baby," Shetty reports. Never fear, though: "HankMed" — aka her boss, Hank (Mark Feuerstein), and his brother Evan (Paulo Costanzo) — "swoop in and help."

But that's just the beginning of Divya's dilemmas. She has also just learned that her platonic roommate, Dr. Sacani (Ben Shenkman), has fallen in love with her. "It's very sweet that people are so enamored by their relationship, but right now Divya's focused on having her first baby by herself," says Shetty. "She's very fond of Dr. Sacani and considers him a valuable friend, but I don't think she's in a state of mind to think about whether that can go anywhere."

Divya has come a long way over the course of Pains' five seasons. "She went from a very conservative girl who was going to have an arranged marriage to maybe a single mother, making her own living away from her parents," says Shetty. "HankMed has become her family. These two brothers have become completely immersed in her life, and she wouldn't know what to do otherwise."

For Shetty, spending five years playing a medical professional has come with its own share of complications. "I was in Saks one day, and this lady fell over unconscious, and nobody was doing anything," she recalls. "I jumped into action and did basic first aid until the paramedics came. It was honestly just pure delusion on my part — I literally felt like it was a scene, then suddenly I realized I was completely unqualified."

Royal Pains airs its season finale Wednesday, Sept. 11 at 9/8c on USA.
