As Colleen's drinking continues to spin out of control, Tommy takes an aggressive approach to get her to stop. Meanwhile, tensions between Franco and Black Shawn boil over, and Mike and Sean attend the ballet with Sean's dying friend Pat (Will Chase).

Tommy loses control after blacking out during a night of heavy drinking, and the crew must put the pieces together the next day. They also must find a missing piece: Colleen.
Season 6, Episode 4
With the cookout approaching one key ingredient is missing, so the crew takes a drastic step to obtain it. Meanwhile, Tommy continues to be troubled by his taste of the afterlife, so he seeks out a priest (Peter Gallagher); Sean helps a cancer-stricken firefighter (Will Chase) break out of the hospital; and Damien consorts with the enemy, so to speak, when he dates a probie (Deanna Russo) from the rival firehouse, the West Side Wildmen.
In this episode of Rescue Me, Tommy's first day back on the job with the boys of 62 Truck turned out to be one of the busiest days in recent memory. It didn't help that the guys were rushing to every call as part of their competition with The West Side Wildmen, the other house in danger of being shut down. Meanwhile, Tommy decided to "stop drinking," by only drinking wine. But as his family became even more frustrated with him, the focus shifted to Lou, who struggled with a dramatic health problem....
In this episode of Rescue Me, Tommy announces his intentions to quit drinking (again), and the idea is met with the usual amount of skepticism from his fellow fire fighters and family members. Tommy's attempts to get his daughter Colleen to do the same, however, end up turning Tommy back to the bottle yet again. Meanwhile, Tommy shares his vision of the afterlife with Lou, but Mike overhears.

In the Season 6 premiere of Rescue Me, Tommy briefly dies after being shot by his Uncle Teddy in the closing moments of Season 5. Before being revived, Tommy sees a terrifying vision of the afterlife that forces him to re-evaluate his life once he's out of the hospital. There's just one problem: Tommy's friends and family have pretty much had it with him, leaving him alone to sort through his troubles.
