Grey's Anatomy Episode Recap: "What a Difference a Day Makes" Season 5, Episode 21

It's Grey's Anatomy's 100th episode and the gang's all heading to the chapel because someone's getting hitched. But is it Meredith and Derek who end up at the altar? While one dynamic duo looks to make their love official, another pair appears to be heading toward Splitsville after a fancy dinner creates some serious tension. Also, George begins to question his future at Seattle Grace.

TODAY'S THE DAY MY LIFE BEGINS

This week, Izzie takes on the narrator role and she's quick to point out, "The days that start out normal ... end up being the biggest." Though there's nothing normal about this day, the day that Meredith and Derek are set to say their "I Do's."

Izzie can barely contain her excitement over the impending Mer/Der nuptials. After all, she has painstakingly planned out every detail, with a little help from Derek's credit card. So what could possibly ruin this perfect day? The return of Denny, which can only mean one thing: a tumor.

Although the MRI came back clean, Derek was able to monitor Izzie's brainwaves during one of her hallucinations, which uncovered the "hidden" tumor. Is it operable? Based on Derek's conversation with Bailey, the outlook isn't good. So, if Derek can't save her life, what's the next best thing he can do for her? Let her have his wedding day!

Derek shares the idea with Meredith, who in turn, gives a heartwrenched Alex the news. While I was close to sharing tears with Karev, the waterworks didn't really kick in until Mer (with a head full of curlers) arrived in Izzie's hospital room to tell her, "This [bridal] dress is not for me!" Poor Izzie thinks Mer's gotten cold feet and is looking for an out. Slowly, Izzie begins to realize what Mer is really saying, just as a handsome Alex arrives in the doorway in his tux. Yep, Izzie and Alex are gonna get hitched!

Feverishly, Mer and Cristina go to work on getting Izzie ready for her big day and soon it's time for the new bride to pick her maid of honor. Cristina gets the gig, as Izzie tells her, "You tried to save my life. That makes you honorable." Cristina is at first stunned, but a look of humbled honor quickly replaces her surprise.

Finally, everyone's at the church, but Izzie finds herself stumbling half-way down the aisle. George rushes to her side to help her the rest of the way. In a symbolic move, George is giving Izzie away to Alex. It's an emotional moment, topped only by Alex's heartfelt and powerful wedding vow, "Today's the day my life begins," he starts, paraphrasing a patient's never-realized graduation-day speech. "Today I become a man. Today I become a husband... I'll be ready for anything...for everything."

And just what might anything and everything end up being for Alex? With Izzie's future in the air and the season finale on tap for next week, he might find out sooner than he thinks.

TOO FANCY

As one couple heads for holy matrimony, another couple prepares to call it quits after a fancy dinner causes a rift between Callie and Arizona.

For the most part, Callie took to avoiding Arizona as much as possible this week by claiming she was too busy to talk. Arizona sensed something was wrong and continually tried to get to the heart of it with Callie, who at one point insinuated work was more invigorating than being with Arizona, "I love the ER! Can't get enough of it. It's fun."

Hurt, Arizona quickly informs Callie that if she wants to end things, that's OK with her: "There are women lining up for me. I am hot!"

Callie then finally confesses it's not Arizona but Callie's financial straits that has her down in the dumps. All through their expensive dinner, Callie was too busy calculating the cost of the meal to enjoy the night. Yep, now that Callie's accounts have been depleted, dinners out aren't really an option anymore. So, the two agree being together is more important than fancy food — and ordering in pizza can be just as good!

GRADUATION DAY

The ER at Seattle Grace was busy this week when a group of college students en route to their graduation ended up in a tragic car accident. George quickly jumps in, but the extent of the injuries is so severe that he ends up losing the first two patients he works on, which quickly shakes his confidence. Feeling like he's in over his head, George soon turns to Owen for help, but finds none and is told to be on his way.

In the end, only one student can be saved — the Valedictorian, who suddenly begins questioning the choices she's made in her life. Should she have slept with her boyfriend (who died as a result of the crash), instead of putting her studies first all this time? At one point, Alex encourages the girl to recite her graduation speech to help get her mind off of everything else and so she begins, "Today's the day my life begins. Today I become a citizen of the world...." As the speech continues, we are presented with the gruesome images of her classmates being zipped up in body bags.

Meanwhile, after so many deaths, George contemplates his future in trauma. Will Owen's suggestion that trauma is a "team sport" keep George on board for this specialty? Time will tell.

GOING SOLO

During an otherwise emotionally charged week, some lighter moments existed when Meredith was presented with her first solo surgery as a wedding gift from Derek. The humorous aspect arrived when the Chief insisted on being in the OR with Mer to oversee things. Slightly offended, Mer quips, "It's my wedding day. I'm not going to kill someone!"

Indeed, the surgery is a success and the Chief tells Mer her mother would be proud.

OWEN AND CRISTINA

While there wasn't much screen time for this duo, it was clear that when the two were together, Owen was reaching out to Cristina (figuratively and literally). However, Cristina was always quick to walk away before Owen could actually clamp his hand down on hers.

FAVORITE LINES:

"Big day. Biggest day of your life. Do you need me to drug you?" - Cristina to Mer

"You just did that because I have cancer. You didn't mean it." - Izzie to Bailey after the two shared a high-five

"Then why aren't you sitting in a corner reading a magazine?" - Mer to the Chief during her solo surgery

"'Cuz we have trauma coming out of our butts!" - Callie

"I don't want to have to be his best man for a third time, if you know what I mean." - Mark to Meredith

"With all the mistakes I've made, I'm lucky the patient's alive." - Mer speculating on what part of her solo surgery would have made her mom proud

"Best non-wedding day ever" - Derek to Mer

"Beautiful, bald Izzie" - Alex lovingly talking to Izzie

"You don't recognize the biggest day of your life ... not until you're in it." - Izzie
