As the day of Colin's brain surgery approaches, Dr. Brown travels with Ephram and Delia to New York to consult with his accomplished mentor, Dr. Donald Douglas (Philip Baker Hall - "Magnolia," "Boogie Nights"). Together, the men devise an innovative method to save Colin--and Dr. Douglas advises Dr. Brown not to perform the extremely risky procedure. When Ephram sees how revered his father is among the students and faculty of his alma mater, he gains a new perspective and makes a major realization. Meanwhile, Colin "kidnaps" Amy and Bright and takes them on an adventure on his last day before the operation, taking care of last-minute emotional tasks. Dr. Brown emerges from the operating room after the grueling surgery and faces Colin's expectant family and loved ones . . .

Meanwhile, as a favor to worried Edna, Dr. Brown accompanies Irv--who would much rather go alone--on a camping trip, forcing Ephram to come along as well and study for his final examinations. At the remote lake, an event occurs that changes father and son forever and enlightens Dr. Brown on how to deal with Colin's obdurate parents. Also, the Abbotts' neighbor, Nina, is overjoyed when her absentee husband, Carl (recurring guest star Dylan Walsh), vows to return home permanently. But a shocking surprise awaits her.

When the owner of Everwood's flower shop dies, Dr. Brown discovers that she was growing marijuana in her backyard to help people who were suffering from cancer. Against Dr. Abbott's wishes, his wife, Rose (recurring guest star Merrilyn Gann), stores the marijuana in their basement until the town decides what to do with it. After much debate, the community decides to burn it. Meanwhile, Dr. Brown's neighbor and friend Nina asks for his advice when a teacher suggests that her son, Sam (recurring guest star Ryan Armstrong), take the therapeutic drug Ritalin. Dr. Brown accompanies Nina to the teacher's meeting and voices his opposition to the idea. But after volunteering to watch Sam for an afternoon, he experiences how unruly Sam really is. Also, Amy and the Harts celebrate Colin's homecoming from the hospital by hosting a party. Colin is overwhelmed and frustrated because he still does not remember most of the people there, including Amy, due to his formerly comatose state.

After the death of his wife, Dr. Andrew "Andy" Brown (series star Treat Williams) trades his career as a world-renowned neurosurgeon for an opportunity to be a better parent and move his two children, 9-year-old Delia (series star Vivien Cardone) and 15-year-old Ephram (series star Gregory Smith) to Everwood, Colorado. The sudden shift is hardest on Ephram, who resents all the years his father lavished on his career at the expense of his family. When Andy opens a free medical clinic for the citizens of Everwood, people think that he is crazy, but a local woman, Edna (series star Debra Mooney), quickly applies for a job as his nurse and office manager. Edna's son, Dr. Harold Abbott, the only other doctor in town, is disturbed by the competition and forbids his children, 15-year-old Amy (series star Emily VanCamp) and 17-year-old Bright from associating with the Brown family. Traditions transform overnight for the residents of Everwood, as recounted by the school bus driver, Mr. Irv.
