You understand how this could work for a while, but for a show that's been around for as long as it has (its 10th season!), you'd expect someone to catch on. That's exactly what happens on Monday's episode, which premieres at 10/9c. What makes it worse is that on the episode, interventions are being planned for two people (enmeshed heroin addict couple Sarah and Mikeal) and so when Sarah figures out what show she's on, she threatens to intervene in Mikeal's intervention by telling him about it in advance. It's riveting — reality TV is rarely this suspenseful.
